摘    要:目的探讨2型糖尿病(2-DM)肾病患者血管性假血友病因子(vWF)的变化及其临床意义.方法采用免疫比浊法测定68例2-DM患者(其中单纯型糖尿病组37例,糖尿病肾病组31例)和20例正常对照组vWF活性.结果糖尿病组(DM)血浆vWF活性明显高于对照组(NC)(P〈0.001),单纯糖尿病组(SDM)与对照组比较差异无显著性(P〉0.05),轻型糖尿病肾病组(MDN)与对照组比较差异有显著性(P〈0.001),重型糖尿病肾病组(SDN)中的vWF活性明显高于MDN组(P〈0.001),亦明显高于单纯性糖尿病组的糖尿病组(P〈0.001).结论vWF在一定程度上可反应糖尿病血管内皮细胞损害情况,vWF水平对糖尿病血管内皮细胞损伤的早期判断及损伤程度判断均有重要的临床参考价值.

Abstract:Objective To investigate the changes and clinical significance of plasma von willebran factor(vWF) in patients with type II diabetic nephropathy(DN).Methods The immunonephelometry assay was use to detect the vWF activity in 68 patients with 2 diabetic mellitus(among there was diabetic nephropathy 31 patients)and 22 healthy subjects.Results The levels of vWF activity in DM group were significant higher than NCgroup(P<0.001),but there was not difference between SDM and NC(P>0.05),the level of vWF activity in mild MDN was higher than NC(P<0.001),and the level of vWF activity in severe SDNgroup was significant higher than single SDM and higher than mild MDNgroup(P<0.001).Conclusion The measurement of plasma vWF activity can reflect the endothelial cell disorders in some extent,the levels of vWF activity is useful in evaluating the early endothelial disorders and the severity of endothelium.
