经皮肝穿胆道置管引流治疗恶性梗阻性黄疸

目的 了解介入治疗恶性梗阻性黄疸的疗效。方法 采用经皮肤穿刺放置内支架或外引流对32例恶性梗阻性黄疸患者进行治疗。结果 中下段胆管梗阻疗效好，治疗一周后多数患者血清胆红素下降超过50％，15天后有16例患者血清胆红素降至正常。结论 胆道介入治疗确实有良好的缓解或消除黄疸的效果，特别是中下段胆道梗阻，黄疸可完全消退，但对上段胆道梗阻的患者，仅能部分缓解黄疸，疗效不如中下段胆道梗阻患者。

TREATMENT OF INTERVENTIONAL PROCEDURE FOR MALIGNANT BILIARY OBSTRUCTION IN 32 CASES

Objective To study the interventional procedure for malignant biliary obstruction.Methods Percutaneous transhepatic stent implantation or external catheter drainage were adopted in 32 patients with malignant biliary obstruction.Results The result was better for common hepatic duct obstruction or cystic duct obstruction. Most patient's TLB in serum decreased more than 50 percent after one week's treatment. The leverl of TLB in 16 cases decreased to normal levels after 10 to 15 days.Conclusion Percutaneous transhepatic stent implantation and biliary drainage are not only simple and effctive treatments but the associated trauma is less than surgery and there are fewer complications.Futhermore, in the treatment of malignant biliary obstruction, these procedures both improved the quality and the length of patient's life.
