老年慢性肺心病急性加重期合并多脏器衰竭病因分析和防治探讨

目的 :了解老年慢性肺心病急性加重期合并多脏器衰竭 (MOSF)的病因 ,探讨临床防治对策。方法 :分析本院 37例慢性肺心病急性加重期合并多脏器衰竭的临床资料 ,结合动脉血气分析、肝功能、肾功能和血液凝血机制的实验室检测全面综合分析。结果 :老年慢性肺心病急性加重期随着年龄增大 ,抵抗力下降 ,出现肺部难于控制的感染 ,长期低氧血症和高碳酸血症 ,导致肺、心、脑、肾、消化道、肝、等多脏器衰竭及DIC ,增加病死率。结论 :老年慢性肺心病急性加重期 ,应加强监测各重要脏器功能的变化 ,对合并多脏器衰竭者 ,关键在于早发现、早诊断、早防治 ,及早控制感染 ,解除气道痉挛 ,改善通气 ,纠正缺氧和高碳酸血症 ,纠正水电解质平衡 ,改善微循环 ,保护肝肾功能 ,提高机体免疫力等综合治疗

Etiologic Factors and Cure of Aged Chronic Pulmonary Heart Disease during Acute Period with Multiple System Organ Failure

Objective: To analyse etiologic factors of aged chronic pulmonary heart disease during actue period,and explore clinical strategy of prevention and cure. Methods: The clinical data of 37 cases with aged chronic pulmonary heart disease during actue period were comprehensively analysed,including arterial blood gas analysis and laboratory monitoring of liver and renal function and coagulation mechanism. Results: Clinical situation showed lower resistance with increasing older age,pulmonary infection and out of control,chrinic hypoxemia and hypercapnia,which led to multiple system organ failure(MSOF),including lung,heart,brain,kidney,digestive tract,liver and DIC,etc. And increased fatality rate. Conclusion: The changes of very important organ function must attract attention. It is important that the steps are takenearly detect,diagnose,prevent and cure,and early control infection,relieve airway spasm,improve ventilation,correct hypoxia and hypercapnia,correct water and electrolytic disorder,improve microcirculation,protect liver and renal function and improve immunity of patients,ect.
