鼻咽癌的CT诊断（附48例报告）

目的分析鼻咽癌的CT表现并评价其在临床诊断与治疗中的意义。方法 48例经病理或穿刺活检证实的鼻咽癌患者均接受了常规CT扫描,5例患者又经对比增强CT扫描。对所有患者的CT表现及临床资料进行了回顾性分析。结果在CT像上,鼻咽癌病灶表现为鼻咽壁增厚（40例）,咽隐窝消失、变平（46例）,鼻咽腔充填软组织肿块（8例）,颅底骨质破坏（5例）,合并颈淋巴结转移（15例）以及合并肝与肺转移（1例）。CT增强扫描显示鼻咽癌病灶呈轻微强化。结论 CT扫描能清楚显示鼻咽癌病灶的部位与范围以及邻近组织的受累情况,同时,它能早期发现颅底骨质破坏,颅内受侵以及淋巴结和远位脏器转移,因此,它在指导临床分期和采取适当的放射治疗中起重要作用。

CT diagnosis of nasopharyngeal carcinonma:a report of 48 cases

Objective To analyze the CT findings of nasopharyngeal carcinoma（NPC） and to evatuate their significance in the clinical diagnosis and treatment.Methods Forty-eight patients with pathologically or clinically puncture biopsy proved NPC underwent conventional CT scans.5 patients underwent additional contrast-enhanced CT scans.The CT findings and clinical data of all the patients were an alyzed retrospectively.Results On CT images,NPC lesions appeared as nasopharyngeal wall thickening（n=40）,pharyngeal recess disappearing and flatting（n=46）,nasopharyngeal cavity filling with soft tissue masses（n=8）,cranio-basilar bone destruction（n=5）,complicated cervical lymph node metastasis（n=15）,and hepatic and pulmonary metastasis（n=1）.Following contrast enhanced CT scans,NPC lesions were lightly enhanced.Conclusion CT scanning can clearly visualize the site and extent of NPC lesions and involvement of neighbour tissues,simultaneously,it can early detect cranio-basilar bone destruction,intracranial invasion,lymph node and remote organ metastasis so that it plays important role in guiding the clinical staging and proper radiotherapy.
