高频超声对小儿肠系膜淋巴结炎的诊断价值

摘    要:目的探讨高频超声对小儿肠系膜淋巴结炎的诊断。方法回顾性分析77例腹痛小儿超声检查结果。采用高频探头超声检查,记录内容包括淋巴结的部位、大小、形态、回声强度及类型,并测量其长短轴比值(L/S),病变的变化结果均经临床和随访证实。结果 77例患儿均显示腹腔淋巴结肿大,脐周及右下腹均见肿大淋巴结者72例,最大者24×12mm,其中19例血流丰富;仅右下腹见肿大淋巴结者4例,最大者14×5mm,1例血流丰富;左上腹及脐周均见肿大淋巴结者1例,大小11×5mm,血流较丰富。治疗后肿大的淋巴结逐渐回缩,但肿大淋巴结的回缩比腹痛消失明显滞后。结论高频超声诊断小儿肠系膜淋巴结炎经济简便,重复性强,准确率高(〉95%),是诊断小儿肠系膜淋巴结炎的首选检查方法,能够为临床诊断、治疗及随访提供重要依据。

Abstract:Objective To evaluate the diagnostic value of high frequency ultrasonography by observing the changes of enlarged lymph nodes in infants and children’s mesenteric lymphadenitis.Methods 77 children with abdominal pain were examined with high frequency ultrasound,and the enlarged lymph nodes were displayed clearly.The position,size,echo,shape and L/S of lymph nodes were enregistered.All the diagnosis were compared with those confirmed by surgical pathology and clinical follow-up.Results 77 children all displayed swelling lymph nodes including 72 cases in periumbilical and right abdominal,4 cases in right abdominal,1 case in left abdominal.The enlarged ymph nodes dwindle in size after therapy,but it appeared later than the disappearence of the abdominal pain.Conclusion High frequency ultrasonography can be the first choice in diagnosis of children’s mesenteric lymphadenitis,the accuracy can achieve 95%.And evaluating the status of the lymph nodes(LNs) has very important value for clinic to diagnose and treat disease.
