嗜铬细胞瘤的超微结构

在81例嗜铬细胞瘤病理分析的基础上,作了10例良性嗜铬细胞瘤(benign pheochromocytoma,BP)和2例恶性嗜铬细胞瘤(malignant pheochromocytoma,MP)的超微结构观察,其中3例做了扫描电镜。发现BP和MP瘤细胞胞浆内的分泌颗粒,明显多于对照组;深电子密度的颗粒尤多。肾上腺髓质内BP和主动脉旁BP,结构基本相同。后者瘤细胞核呈不规则型为多;核内见中等密度、园形、均质的脂滴样小体。2例青少年的MP椎体转移灶。均为肾上腺外瘤(膀胱和主动脉旁),瘤细胞呈明显异型性,BP和MP与血管关系密切,瘤细胞绕于毛细血管,小血管周围,分泌激素可见有内皮细胞逆向吞饮;血窦和小血管为弥散和管道而入血。

Study on Ultrastructure of Benign and Malignant Pheochromocytoma

On the basis of 81 cases of pheochromocyoma reported previously, an ultrastructural study of 12 of those cases (10 benign and 2 malignant) were further made, and 3 of the 12 benign cases were examined with scanning eletronic micro- scope. The results showed: the cell plasm of benign and malignant pheochromocytoma contains obviously more endosecretory granules than that of the control. There is no obvious difference in structure between intra-adrenal and extra-adrenal benign pheochromocytoma. However, the nuclei of the extra-tumors contain lipoid like droplets. Two extra-adrenal malignant tumors both found in youth have vertebral metastasis. Tumor cells in both benign and malignant pheochromocytoma always exist around capillaries and vessels. Endosecretory granules can be found in endothelium cell plasm, vascular sinus and small vessels.
