慢性阻塞性肺疾病肺通气与血浆TXB_2、6-K-PGF_(1a)及免疫状态的关系

摘    要:对85例慢性阻塞性肺疾病(COPD)患者进行了1s率(FEV1%),最大呼气中期流速(MMEF),血栓素B2(TXB2),6-酮-前列腺素F1a(6K-K-PGF1a),T淋巴细胞总数(T3)及亚群(T4,T8),IgA、G、M测定。结果:FEV1%降低,血浆中TXB2浓度升高,TXB2/6-K-PGF1a比值增大,外周血中T4减少,T8增多,T4/T8比值下降。MMEF重度降低时,TXB2和TXB2/6K-K-PGF1a升高,IgG改变显著,T4减少,T4/T8比值降低。T4/T8比值降低患者,血浆中TXB2浓度升高,TXB2/6-K-PGF1a比值增大,差异显著。结果提示:COPD患者可能有TXB2/6-K-PGF1a平衡紊乱,免疫功能改变,T4/T8比值改变可能影响TXB2/6-K-PGF1a平衡。大气道和小气道发生和发展机制不完全相同。

Abstract:FEV1 % MMEF, plasmatic TXB2and 6-K-PGF1a, total number of T lymphocyte(T3) and T-lymphocyte subsets (T4, T8 ), and plasmatic IgA. G. M were determined in eight-five chronic obstruction pulmonary disease patients. The results revealed that when FEV1 % Lowered, the plasmatic concentration of TXB2 and the ratio of TXB2/6-K-PGF1a increased, the number of T4 decreased and the number of T8 increased with the low ratio of T4/T8, that the TXB2 elevated with the high ratio of TXB2/6-K-PGF1a, IgG increased, and the number of T4 and the ratio of T4/T8 decreased in seriously lowered MMEF group and that TXB2 and the ratio of TXB2/6-K-PGF1a increased in lowered T4/T8. The findings suggested that there might be TXB2 and 6-K-PGF1a imbalance and immunological changes in COPD patients, and that the ratio of T4/T8 might affect the balance of TXB2/6-K-PGF1a and there were different roles between large airways and small airways in pathogenesis of COPD.
