颈椎三维活动度测定仪的测量稳定性评估

摘    要:目的对自行设计的颈椎三维活动度测定仪的测量稳定性进行评估。方法由A、B医师分别对30名健康受试者的颈椎屈伸、侧屈、旋转活动度进行2次重复测量,得出本组受试者颈椎三维活动度的平均值和95%可信区间(confidence interval,CI),得出A医师重复测量的组间相关系数(intraclass correlation coefficient,ICC)和95%CI,得出A医师与B医师间重复测量的ICC和95%CI。结果本组受试者的颈椎活动度均值为:屈伸123.0°±8.3°,侧屈86.3°±8.9°,旋转135.1°±8.3°。观察者内屈伸活动度一致性非常好(ICC=0.87),侧屈活动度高度一致(ICC=0.92),旋转活动度高度一致(ICC=0.94);观察者间屈伸活动度一致性非常好(ICC=0.87),侧屈活动度高度一致(ICC=0.90),旋转活动度高度一致(ICC=0.91)。结论本实验证实颈椎三维活动度测定仪具有良好的观察者内信度和观察者间信度,是一种简单实用、稳定精确的颈椎活动度测量工具,适合临床和科研使用。

Abstract:Objective To evaluate the stability of 3D cervical range of motion(CROM) inclinometer. Methods Thirty healthy subjects underwent the active cervical ROM testing in the 3 planes using the 3D CROM inclinometer. Observer A and B evaluated the cervical ROM of each subject twice respectively, and determined the mean cervical ROM in flexion-extension, lateral flexion and rotation in these subjects, and examined the intra-observer and inter-observer reliability of measurements obtained. Results In these subjects, the mean cervical ROM was 123.0°± 8.3° in flexion-extension, 86.3°± 8.9° in lateral flexion and 135.1°± 8.3° in rotation. Good intra-observer reliability was found for flexion-extension (ICC=0.87), high intra-observer reliability was found for lateral flexion (ICC=0.92)and rotation (ICC=0.94). Good inter-observer reliability was found for flexion-extension (ICC=0.87), high intra-observer reliability was found for lateral flexion (ICC=0.90)and rotation (ICC=0.91). Conclusion The 3D CROM inclinometer is a reliable instrument for determining active cervical ROM with good intra-observer and inter-observer reliability. It is a practical and valid instrument appropriate for clinical evaluation and research.
