中药胰必清对急性坏死性胰腺炎兔微循环障碍的治疗作用

摘    要:目的 :探讨中药胰必清对急性坏死性胰腺炎 (ANP)兔微循环障碍的治疗作用。方法 :逆行胰管注射 5 %的牛磺胆酸钠制作急性坏死性胰腺炎模型。 30只日本大耳兔随机分为假手术组 (A组 ,15只 )、ANP组 (B组 ,15只 )、ANP加中药治疗组 (C组 ,15只 )。观察各组血清酶学、血液流变学的改变 ,以及胰腺、回肠组织超氧化物歧化酶 (SOD)、黄嘌呤氧化酶 (XO)和丙二醛 (MDA)的含量 ,胰腺、肠粘膜的病理学形态改变。结果 :术后第 3d和第 7d ,C组血清淀粉酶水平明显低于B组 ;B组全血低切粘度、血浆粘度、红细胞压积、红细胞聚集指数分别为 8.17±0 .5 6 ,2 .6 1± 1.5 4 ,5 3.38± 13.2 0 ,2 .6 8± 1.32 ,而C组分别为 6 .5 7± 1.33,1.30± 0 .35 ,4 0 .0 1± 7.12 ,2 .2 0± 0 .5 0 ,与B组比较差异有显著性 (P <0 .0 5 ) ;C组胰匀浆和肠匀浆SOD的含量分别为 (2 80 .38± 2 8.5 0 )ng·L- 1 和 (2 94 .4 1± 9.6 5 )ng·L- 1 ,明显高于B组 (174 .5 7± 14 .4 0 )ng·L- 1 和 (112 .76± 11.2 4 )ng·L- 1 (P <0 .0 1) ,而C组胰匀浆和肠匀浆MDA的含量分别为 (8.5 2± 0 .80 ) μmol·L- 1 和 (6 .90± 0 .2 5 ) μmol·L- 1 ,明显低于B组 (11.38± 1.73) μmol·L- 1 和 (9.2 6± 0 .2 5 )μmol·L- 1 (P <0 .0 1) ,C组?

Abstract:Objective: To investigate the therapeutic effects of Chinese Medicine yibiqing on microcirculation dysfunction in acute necrotizing pancreatitis(ANP) in rabbits. Methods: Retrograde pancreatic duct injection of 5% sodium taurocholate 2.0 ml·kg -1 was used to establish ANP models. All rabbits were randomly divided into three groups: the sham operative group(Group A,n=15),the ANP group(Group B,n=15), and the ANP with Chinese Medicine group(Group C,n=15). The levels of serum/plasma amylase,the changes of blood rheology, the contents of superoxide dismutase(SOD), xanthine oxidase(XO) and malondialdehyde(MDA) in pancreas and ileum and the pathology of pancreas and intestinal mucosa were examined. Results: In the third and seventh day postoperation, the levels of serum amylase in Group C were significantly lower than those in Group B; in Group C, the blood rheology indexes were lower than those in Group B(the whole blood viscosity at low shear rate was (6.57±1.33) vs (8.17±0.56),P<0.05; the plasma viscosity was (1.30±0.35) vs (2.61±1.54),P<0.05; the hematocvit was (40.01±7.12) vs (53.38±13.20),P< 0.05 ;the agglutination index of red blood cells was (2.20±0.50) vs (2.68±1.32),P<0.05; in Group C,the contents of SOD in pancreas and ileum were significantly higher than those in Group Bpancreas:(280.38±28.50) ng·L -1 vs (174.57±14.40) ng·L -1 ,P<0.01,ileum: (294.41±9.65)ng·L -1 vs (112.76±11.2) ng·L -1 ,P<0.01],but the contents of MDA and XO in pancreas and ileum were significantly lower than that in Group BMDA in pancreas: ( 8.52 ± 0.80 )μmol·L -1 vs(11.38±1.73)μmol·L -1 ,P<0.01, MDA in ileum: (6.90±0.25)μmol·L -1 vs(9.26±0.25)μmol·L -1 ,P<0.01,XO in pancreas: (109.32±11.54)ng·L -1 vs (170.21±7.13) ng·L -1 ,P<0.05, XO in ileum:(120.35±16.10) ng·L -1 vs (182.54±17.12 )ng·L -1 ,P<0.01]. Conclusion: Chinese Medicine yibiqing can ameliorate the microcirculation dysfunction in acute nectrotizing pancreatitis in rabbits.
