不开胸食管剥脱术在食管贲门癌手术治疗中的应用

目的 为扩大外科治疗食管癌治疗范围，减少合并症，提高手术效果。方法 对52例食管、贲门癌患者进行了不开胸食管剥脱术，切除食管癌以胃作替代器官，行胃食管吻合术。结果 全组患者均获切除，无手术死亡，术后发生颈部吻合口瘘3例(5．9％)，喉返神经暂时性麻痹5例(9．6％)。结论 对于年龄较大、心肺功能较差、适当病期的肿瘤采用不开胸食管剥脱术治疗食管癌创伤小，并发症少，效果好。

Application of esophagectomg without thoracotomy in surgical treatment of esophagocardial cancer

Objective To extent the range of surgical therapy for esophageal carcinoma to decrease the complication of operation and to improve the effect of operation. Methods 52 patients with esophageal or esophagocardial carcinoma were subjected to esophagectomy without thoracotomy and esophagogastrostomy. Results No patients was died due to operation. The cervical fistula occurred in 3 cases ( 5.9%) and temporally recurrent nerve injury in 5 cases ( 9.6%) after operation. Conclusion The therapy of esophagectomy without thoracotomy has better effect and less complications to the old patients with abnormal heart and pulmonary functions.
