强直性脊柱炎脊柱骨折的影像学表现及损伤特点

目的探讨强直性脊柱炎（AS）脊柱骨折的多种影像学特征及损伤特点。方法回顾性分析近年来我院收治的17例明确强脊炎患者合并脊柱骨折病人的影像检查和临床资料，总结其特点。结果17例患者中，颈椎骨折3例，胸腰段骨折14例，其中剪力骨折3例，应力骨折13例，胸腰段多发压缩骨折1例，贯通三枉骨折15例，累及椎体32个，椎间关节38处，合并脊髓损伤5例。结论AS患者脊柱骨折通常表现为累及三柱的贯通骨折，其中应力骨折最为多见，影像学检查是发现病变的主要手段，MRI检查对显示脊髓损伤及椎体终板病变具有明显优势。

Imaging Manifestations and Injury Characteristics of Ankylosing Spondylitis with Spinal Fracture

Objective To explore kinds of imaging manifestations and injure characteristics of ankylosing spondylitis（AS） with spinal fracture. Methods hnages and clinical data of 17 patients with AS with spinal fiacture were analyzed retrospectively. Result In the all 17 cases, 3 cases were cervical fi＇acture, 14 cases were thoracic and lumbar spinal fracture. 3 cases were shearing fracture： 13 cases were stress fracture. 1 case wax multiple compres- sion fractures in thoracic and lumbar spine. Three columns of the spine were involved in 15 patients, involving 32 vertebral bodies and 38 inter-vertebral joints, and complicated with spinal cord injury in 5 patients. Conclusion Patients with AS always showed three col- umns fracture, in which stress fi＇acture is most common, hnaging examination is the main means to find lesions. MRI has obvious advantages in showing spinal cord injury and vertebral endplate lesions.
