应变率成像评价糖尿病患者右室舒缩功能

摘    要:目的 探讨应变率成像评价亚临床状态的糖尿病患者右室功能的价值.方法 糖尿病患者30例、糖尿病合并高血压患者30例、健康对照组30例,应用应变率成像技术及二维超声心动图检测其右室功能.结果 糖尿病组及糖尿病合并高血压病组右室游离壁基底段、心尖段收缩期峰值应变率(SRs)、舒张早期应变率(SRe)、舒张早期应变率/舒张晚期应变率(SRe/SRa)较对照组减低,差异有统计学意义(P〈0.05).糖尿病组右室游离壁基底段、心尖段SRs、SRe、SRe/SRa与糖尿病合并高血压病组比较,差异无统计学意义(P〉0.05).各组之间右室射血分数、三尖瓣血流舒张早期峰值速度/舒张晚期峰值速度差异无统计学意义(P〉0.05).结论 应变率成像可无创性定量评估糖尿病患者右室舒缩功能,为亚临床状态的糖尿病患者右室功能的评估提供了一种可靠方法.

Abstract:Objective To assess the value of right ventrieular systolic and diastolic function in subclinical patients with diabetes mellitus(DM) by strain rate imaging. Methods Thirty patients with diabetes mellitus (DM) ,30 patients with diabetes mellitus and hypertension ( DM + HE) and 30 healthy subjects were enrolled in this study. Their right ventrieular systolic and diastolic function were detected by strain rate imaging and two - dimensional eehocardiography. Results Compared with the control group, the peak systolic strain rate (SRs) of apical segment of right ventricular free wall was significantly lower in patients with pure DM and DM + HE( P 〈 0.05 ), the peak systolic strain rate (SRs) of basal segment, peak early diastolic strain rate (SRe) and the ratio of peak early diastolic strain rate and peak late diastolic strain rate ( SRe/SRa ) of basal and apical segment were significantly lower in patients with pure DM and DM + HE( P 〈 0.05 ). These was no significant difference of SRs, SRe and SRe/ SRa between DM and DM + HE group( P 〉 0.05 ). These was no significant difference of right ventrieular ejection fraction (EF) and ratio of peak early diastolic velocity and peak late diastolic velocity of tricuspid flow among control group, DM and DM + HE group ( P 〉 0.05 ). Conclusion Strain rate imaging can non - invasively and quantitatively assess right ventricular systolic and diastolic function in patients with diabetes mellitus. It is a reliable method to assess right ventrieular function in subclinical patients with diabetes mellitus.
