胸腺瘤的CT分析及诊断价值

目的探讨胸腺瘤的CT诊断价值。方法回顾性分析经手术病理证实的26例胸腺瘤，对其CT平扫及增强资料进行分析。结果非侵袭性胸腺瘤13例，瘤体边缘规则，密度均匀，无纵隔胸膜和心包受侵征象。侵袭性胸腺瘤12例，瘤体边缘不规则，与心脏血管接触面大，脂肪间隙消失，有不同程度纵隔铸形改变；其中10例胸膜和心包受侵、增厚；9例有胸水和心包积液；2例侵入肺内；2例纵隔淋巴结肿大。胸腺癌1例，瘤体沿纵隔血管间隙侵润生长，伴纵隔淋巴结和胸膜转移。结论CT能清晰显示胸腺瘤形态和内部结构及周围侵犯情况，具有重要的诊断和鉴别诊断价值。

Diagnosis Value of CT on thymoma

Objective To study the value of CT on diagnosis of thymoma. Methods We reviewed and analyzed 26 patients with thymoma by plain and enhancement CT scan. The patients were confirmed by surgery and pathology. Results There were 13 patients with non-invasive thymomas, which demonstrated clear margin, uniform density, non-invasion of mediastinal pleural and pericardium with CT scanning. There were 12 patients with invasive thymomas, which demonstrated irregular margin, largely contact with cardiovascular, fat space disappear and some degrees of mediastinal mold change with CT. Conclusion CT has important diagnostic and differential diagnostic value, which can clearly show the shape, internal structure of thymoma and violations around the tumor.
