摘    要:目的:探讨术前超声定位在微型腹腔镜甲状腺切除术前后的可行性和临床实用价值。方法:对30例甲状腺结节患者,测量结节与气管内侧缘的距离、与前包膜的距离、与上下极平面的距离;并对手术的结节切除率、手术时间、术中出血量、住院时间进行分析。结果:29例患者于腔镜术中按超声所测数据确定的位置均能迅速找到结节,结节切除率97.2%,除1例发生一过性声音嘶哑外,无一例发生严重并发症。结论:术前超声检查有助于了解甲状腺结节及其周围情况,术前测量定位数据有利于微型腹腔镜甲状腺切除术中迅速找到并切除结节,是一种安全可行和预防并发症发生的有效途径。

Abstract:Objective:To discuss and study the feasibility and application value of microlaparoscopic thyroidectomy by ultrasonic orientation before surgery.Methods:30 cases of patients with thyroid node,measuring the interval(distance) from the node to the trachea and its internal brim,the interval to front envelope,and the interval to the plane of the upper and lower ends.No orientation was performed to patients in the contrast group.Compared and analyzed the resecting rate,operation time,hemorrhages during surgery and hospitalizing time.Results:Ultrasonic orientation helped to find hypothyroid focuses quickly for successful resection for 30 patients to be undertaken thyroiddectomy.The node desecting rate reached 97.2%.No serious complications occurred except that one case encountered one-off rustiness.Conclusions:Ultrasonic orientation was helpful in detecting hypothyroid node and its surrounding prior to surgery.Data of measuring and orientation prior to surgery was propitious to find out hypothyroid focuses quickly for resection in micro laparoscopic thyroidectomy.It is a safe,feasible and effective way for this kind of surgery to prevent complications.
