血液透析性低血压的相关因素分析

目的：分析透析性低血压的相关因素。方法：根据透析性低血压诊断标准，将本院维持性血透患者60例分为低血压组（A组）30侈4和非低血压组（B组）30例，比较两组间原发病因、透龄、透析前干体重（DBW）、超滤量（UF）、UF门DBW、血红蛋白（Hb）、血浆白蛋白（Alb）、C反应蛋白（CRP）和左室质量指数（LVMI）等资料的差异。结果：A组uF和DBw，UF均明显高于B组（t=2．257，P〈0．05，t=3．98，P〈0．01）；A组Hb、AJb水平明显低于B组（t=3．535，P〈0．01，t=2．095，P〈0．05）；A组CRP、LVMI水平明显高于B组（t=5．500，t=7．221，P均〈0．01）；A组与UF、UF/DBW、CRP、LVMI呈显著正相关（r=0．258，P〈0．05，r=0-314，r=0．458，r=0．358，P均〈0．01），与Hb、Alb呈显著负相关（r=-0.348，P〈0．01，r=-0．269。P〈0．05）。结论：严重贫血、低蛋白血症、炎症状态、左室肥厚及超滤量过多均与透析性低血压的发生密切相关。

Analyze the related factors of Hemodialysis Associated Hypotension

Objective To analyze the related factors of hemodialysis associated hypotension.Methods According to the diagnostic standard of hemodialysis associated hypotension, 60 patients with maintained hemodialysis were divided into two groups： low blood pressure group （group A） 30 eases and no low blood pressure group （group B） 30 cases,The dialysitic duration,body dry weight（DBW）,ultrafiltration（UF）,UF/DBW、 hemoglobin（Hb）,albumin（Alb）,c-reantive protein（CRP）, and left ventricular mass index（LVMI）, were compared between two groups.Results The contents of UF and DBW / UF in group A were significantly higher than the group B （t=2.257,P〈0.05, t=3.98, P〈0.01）; The contents of Hb, Alb in group A were significantly lower than the B group （t= 3.535, P〈 0.01, t= 2.095, P〈0.05）;The index of CRP, LVMI in group A were significantly higher than the B group （t=5.500,t=7.221,all P〈0.01）; The index of UF, UF / DBW, CRP, LYMI in group A showed significant positive correlation （r = 0.258, P 〈0.05, r =0.314, r= 0.458, r=0.358, all P〈0.01）, and Hb, Alb in group A showed significant negative correlation （r=-0.348, P〈0.01,r=- 0.269,P〈0.05）. Conclusion Severe anemia, hypoalbuminemia, inflammation, left ventrieular hypertrophy and excessive ultrafiltration and dialysis hypotension were closely related to the occurrence.
