经阴道超声与腹部超声诊断宫腔内部分妊娠组织残留的对照研究

目的探讨宫腔内部分妊娠组织残留的阴道超声与腹部超声诊断价值。方法回顾性分析2011年1月-2012年1月在本院行流产或引产术后超声复查的92例患者的临床资料,根据检查方法不同分为观察组52例,对照组40例。观察组行阴道超声检查,对照组行腹部超声检查。比较检查方法对宫腔内部分妊娠组织残留的诊断价值。结果观察组诊断为宫腔内部分妊娠组织残留52例,经清宫术后病理证实50例,误诊2例,符合率为96.15%;对照组检出40例,病理证实35例,误诊5例,符合率为87.5%。彩色多普勒显示宫壁组织条状、丛状、短线状或点状血流信号,不同超声检查方法对宫内部分妊娠残余组织检出符合率比较,χ2=6.853,P〈0.01,提示组间比较存在显著性差异,有统计学意义。结论在诊断宫腔内部分妊娠组织残留方面,阴道超声检查诊断符合率显著高于腹部超声检查的诊断符合率,阴道超声检查有助于在流产后及时诊断宫内妊娠组织残留,从而减少流产并发症,值得在临床诊断中应用。

Control Study of Transvaginal Ultrasound and Abdominal Ultra-sound in the Diagnoses of Some Pregnancy Tissue Residues

Objective To investigate the value of transvaginal ultrasound and abdominal ultrasound in the diagnoses of some pregnancy tissue residues. Methods 92 patients in our hospital from 2011.1 to 2012.1 was retrospectively analyzed. According to the inspection method, they were divided into observation group of 52 patients and control group of 40 patients; Transvaginal ultrasound was used in observation group. Abdominal ultrasound was used in control group. The diagnostic value of Some pregnancy tissue remaining in intrauterine was compared. Results In observation group diagnosed as intrauterine pregnancy tissue remaining was 52 cases section, 50 cases confirmed by curettage pathology, 2 cases of misdiagnosis, the rate was 96.15%. In the control group, 40 cases were detected, 35 cases were confirmed by pathologically, 5 cases of misdiagnosis, the rate was 87.5%. Flow signals of strips, plexiform, short linear or dot were showed by Color Doppler. Coincidence detection of some residual tissue intrauterine pregnancy by different ultrasound methods were compared, χ2=6.853,P 〈0.01, there were statistically significant between the groups. Conclusion In the diagnosis of some pregnancy tissue residues in intrauterine, diagnosis rate of transvaginal ultrasound was significantly higher than abdominal ultrasound, helping to timely diagnose pregnancy tissue residues in intrauterine after early post-abortion, reducing the complications of abortion, it is worth of application in clinical diagnosis.
