腹腔镜胃十二指肠溃疡穿孔修补术40例临床观察

目的探讨腹腔镜胃十二指肠溃疡穿孔修补术的临床应用价值。方法将2008年6月～2012年1月在本院行腹腔镜胃十二指肠溃疡穿孔修补术的40例纳入本研究，设为观察组，另选择同期行开腹胃十二指肠溃疡穿孔修补术的40例患者作为对照组。结果观察组40例患者手术时间明显短于对照组，术后患者排气时间早，下床活动早，且住院时间短于对照组（P〈0．05）。观察组并发症发生率为5．0％，明显低于对照组（15．0％）。随访1年内，观察组全部痊愈，无一例复发，对照组39例痊愈，复发1例（2．5％），两组复发率比较，差异无统计学意义（P〉0．05）。结论腹腔镜胃十二指肠溃疡穿孔修补术明显优于传统开腹手术，具有手术时间短，患者术后恢复快，住院时间短，并发症少等优点，值得广泛推广和应用。

Clinical observation of laparoscopic gastric ulcer perforation repair in 40 cases

Objective To explore the clinical application value of laparoscopic gastric ulcer perforation repair.Methods Forty patients with laparoscopic gastric ulcer perforation repair in our hospital from June 2008 to January 2012 were selected as observation group,and 40 patients with open gastric ulcer perforation repair surgery at the same period were selected as control group.Results The operative time of observation group was significantly shorter than that of the control group,the time of anus exhaust and out of bed were earlier,and hospital stay of observation group was shorter than those of the control group(P < 0.05).The complication rate of observation group was 5.0%,which was significantly lower than that of the control group(15.0%).Followed-up for one year,patients in observation group were all recoveried and no recurrence occured,the control group were recoveried of 35 cases,5 cases were recurrent(12.5%),the difference of the recurrence rate between two groups was not significant(P > 0.05).Conclusion Laparoscopic gastric ulcer perforation repair is superior to traditional open surgery.It has the advantage of shorter operative time,rapid postoperative recovery,shorter hospital stay,fewer complications,ect.,so it is worthy of wide promotion and application.
