手术治疗甲状腺微小乳头状癌预后及其影响因素分析

目的分析手术治疗甲状腺微小乳头状癌（PTMC）预后及其影响因素。方法回顾性分析本院手术治疗的128例PTMC患者的临床和随访资料,总结分析患者术后预后情况;根据随访生存情况,将患者分为死亡组和非死亡组,比较两组患者相关参数的差异。结果 16例患者术后出现并发症,其中,声带麻痹7例,甲状旁腺功能减退9例;18例患者术后肿瘤复发,其中,局部区域复发15例,肺转移患者3例;术后5年生存率为93.75%（120/128）。死亡组患者年龄、有淋巴结转移比例和远处转移比例显著高于非死亡组（P〈0.05）;两组患者在性别、病程、肿瘤大小、手术方式、肿瘤复发方面差异无统计学意义（P〉0.05）。结论手术治疗是PTMC患者较理想的选择,年龄、淋巴结转移和远处转移可能是影响其预后的因素。

Analysis of the prognosis and impact factors of papillary thyroid microcaricinoma treated by surgery

Objective To analyze the prognosis and impact factors of papillary thyroid microcaricinoma treated by surgery.Methods The clinical and following-up data of 128 patients with papillary thyroid microcaricinoma treated by surgery were analyzed retrospectively.According to survival,all patients were divided into death group and non-death group.The parameters were compared between two groups.Results Among the 16 patients with postoperative complications,7 patients were found with vocal cord paralysis,9 patients were found with hypoparathyroidism.Among the 18 patients with tumor recurrence,15 patients were found with locoregional recurrence,3 patients were found with lung metastases.The 5-year survival rate of 128 patients was 93.75%(120/128).Compared with non-death group,the age,lymph node metastasis and distant metastasis of death group had significant differences(P < 0.05).There were not significant differences between two groups in sex,tumor size,surgical approach and tumor recurrence(P > 0.05).Conclusion Surgical treatment is suggested to be a good treatment for those patients.The factors of age,lymph node metastasis and distant metastasis may impact the prognosis.
