寰枢椎脱位经口咽入路减压手术感染的预防

目的探讨寰枢椎脱位经口咽入路减压手术感染的预防方法与效果。方法寰枢椎脱位患者40例根据患者意愿分为治疗组与对照组各20例，两组都采用寰枢椎脱位经口咽人路减压手术，治疗组患者在手术治疗的同时积极预防感染。结果两组治疗后JOA评分都有明显提高（P〈0．05），两组治疗前后对比，差异均无统计学意义（P〉0．05）。对照组术后感染发生率为20．0％，治疗组未发生感染，两组感染率比较，差异有统计学意义（X^2=12．3692，P〈0．05），治疗组明显少于对照组。结论寰枢椎脱位经口咽入路减压手术能取得很好的预后效果，但是要积极预防感染，保证患者平稳度过围术期。

The infection prevention of atlantoaxial dislocation by transoral transpharyngeal approach decompression surgery

Objective To investigate the infection prevention ways and effects of atlantoaxial dislocation by transoral transpharyngeal approach decompression surgery.Methods Forty cases of patients with atlantoaxial dislocation were equally divided into treatment group and control group according to the wishes of patients,two groups were adopted the atlantoaxial dislocation by transoral transpharyngeal approach decompression surgery,and the patients in the treatment group received the aggressive infections prevention.Results After treatment,the JOA score of the two groups were both improved significantly(P < 0.05),before and after treatment in both groups had no significant difference(P > 0.05).In the control group,the incidence of infection was 20.0%,the treatment group had no infections,the rate of infection between two groups had significant difference(χ2 = 12.369 2,P < 0.05).Conclusion Atlantoaxial dislocation by transoral transpharyngeal approach decompression surgery can achieve good prognosis,but doctors should adopt aggressive infection prevention to ensure stable perioperative periods for patients.
