米力农对严重慢性心力衰竭患者血清基质金属蛋白酶-9及B型钠尿肽水平的短期影响

摘    要:目的观察米力农对严重慢性心力衰竭(CHF)患者血清基质金属蛋白酶-9(MMP-9)及B型钠尿肽(BNP)水平的影响。方法将66例严重CHF患者随机分为常规治疗组(n=32)和米力农治疗组(n=34),后者在常规治疗基础上加用米力农,于治疗7d前后,采用酶联免疫吸附法(ELISA)测定血清MMP-9及B型钠尿肽的水平,采用经胸超声心动图测定左心室射血分数(LVEF)。结果治疗7d后,米力农治疗组总有效率为91.18%(31/34),显著高于常规治疗组[62.50%(20/32)](P〈0.01);米力农治疗组血清BNP及MMP-9水平较治疗前显著下降(P〈0.01),并显著低于常规治疗组(P〈0.01);米力农治疗组LVEF值显著高于治疗前及常规治疗组(P〈0.01)。结论常规治疗基础上短期应用米力农,能够显著改善严重慢性心力衰竭患者的心功能,并能显著降低血清MMP-9及BNP水平。

Abstract:Objective To explore the short term effect of milrinone on serum matrix mmetalloproteinase-9(MMP-9) and brain natriuretic peptide(BNP) in patients with severe congestive heart failure.Methods Sixty-six patients with chronic heart failure(CHF) were divided into routine therapy group(n = 32) and milrinone therapy group(n = 34),milrinone therapy equal to routine therapy plus milrinone,before and after 7 days therapy,plasma MMP-9 and BNP was measured by ELISA,the left ventricular ejection fraction(LVEF) was measured by echocardiography.Results After 7 days therapy,total efficient rates was 91.18%(31/34),it was higher significantly in milrinone therapy group than routine therapy group [62.50%(20/32)](P < 0.01);The plasma MMP-9 and BNP level was lower significantly in milrinone therapy group than before therapy(P < 0.01),and lower significantly than routine therapy group(P < 0.01);LVEF was higher significantly in milrinone therapy group than routine therapy group and before therapy(P < 0.01).Conclusion Short term of milrinone therapy can improve significantly cardiac function in patients with severe heart failure,and reduce significantly plasma BNP and MMP-9 levels.
