经皮肾镜取石术和后腹腔镜输尿管上段切开取石术治疗复杂性输尿管上段结石的临床对比分析

目的比较分析经皮肾镜取石术和后腹腔镜输尿管上段切开取石术对复杂性输尿管上段结石的临床疗效。方法 180例输尿管上段结石患者随机平分为治疗组与对照组,各90例,治疗组行经皮肾镜取石术,对照组行后腹腔镜输尿管上段切开取石术。结果治疗组的手术时间、出血量和住院时间都高于对照组,但是差异无统计学意义（P〉0.05）。治疗组的并发症发生率明显少于对照组（P〈0.05）,结石清除率明显高于对照组（P〈0.05）。结论经皮肾镜取石术和后腹腔镜输尿管上段切开取石术治疗复杂性输尿管上段结石各有利弊,需在熟练操作的前提下,严格掌握适应证,合理选择手术方式。

Clinical comparison analysis of percutaneous nephrolithotomy lithotomy and retroperitoneal laparoscopic ureterolithotomy in the treatment of complicated upper ureteral calculi

Objective To compare the clinical effects between percutaneous nephrolithotomy lithotomy and retroperitoneal laparoscopic ureterolithotomy in the treatment of complicated upper ureteral calculi. Methods One hundred and eighty cases of patients with upper ureteral calculi were selected, and randomly divided into the treatment group and control group, each group had 90 cases. The treatment group was given percutaneous nephrostomy lithotomy treatment, the control group was given retroperitoneal laparoscopic ureterolithotomy treatment. Results The operative time, amount of bleeding and hospital stays of the treatment group were higher than those of the control group, but the differences were not statistically significant (P > 0.05). The incidence of complications in the treatment group was lower than that of the control group (P < 0.05), and the calculi clearance rate of the treatment group was significantly higher than that of the control group (P < 0.05). Conclusion Both percutaneous nephrolithotomy lithotomy and retroperitoneal laparoscopic ureterolithotomy in the treatment of complicated upper ureteral calculi have its advantages and disadvantages. Doctors should master indications strictly and select reasonable surgical approach, on the condition of skilled operation.
