高甘油三酯血症性急性胰腺炎9例临床诊治分析

目的分析高甘油三酯血症性急性胰腺炎患者的临床诊治,旨在加强对高甘油三酯血症合并急性胰腺炎发病类型及其诊治的认识。方法随机选择2009年5月～2011年5月本院消化内科收治的急性胰腺炎患者24例为研究对象,分为两组,其中高甘油三酯血症组为9例,胆源性急性胰腺炎组为15例。结果高脂血症合并急性胰腺炎的患者更容易发生重症性急性胰腺炎,且其病死率显著高于胆源性急性胰腺炎。高甘油三酯血症性急性胰腺炎患者易发生多器官衰竭,同时血淀粉酶与胆源性急性胰腺炎相比明显降低。结论高甘油三酯血症急性胰腺炎患者由于＂三高＂等原因,更易导致患者发生重症急性胰腺炎甚至死亡,应对此类患者加强饮食、减重、降脂的控制。

Analysis of clinical diagnosis and treatment of hypertriglyceridemia-induced acute pancreatitis in 9 cases

Objective To analyze the clinical diagnosis and treatment of hypertriglyceridemia-induced acute pancreatitis, in order to strengthen the cognition of pathogenesis type, diagnosis and treatment of hypertriglyceridemia combined with acute pancreatitis. Methods Twenty-four cases of patients with acute pancreatitis treated in GI medicine department of our hospital from May 2009 to May 2011 were randomly selected as the research objects, and divided into two groups. There were 9 cases in the hypertriglyceridemia group and 15 cases in the biliogenic acute pancreatitis group. Results Acute pancreatitis patients combined with hypertriglyceridemia were more prone to severe acute pancreatitis, and the fatality rate was significantly higher than patients with biliogenic acute pancreatitis. Acute pancreatitis patients combined with hypertriglyceridemia were susceptible to multiple organ failure, meanwhile whose blood amylase level was significantly lower than patients with biliogenic acute pancreatitis. Conclusion Acute pancreatitis patients combined with hypertriglyceridemia easily lead to severe acute pancreatitis and even death by reasons of "three high" and so on. Therefore, diet, weight reduction and lipid-loweringing should be controlled for these patients.
