腹部创伤130例临床诊断及治疗分析

目的探讨腹部创伤的临床诊断及治疗。方法回顾性分析2006年6月～2011年6月本科收治的腹部创伤患者130例的临床资料，对其致伤原因、伤情分布、损伤严重程度（ISS）评分、诊断经过及手术治疗方法进行分析。结果本组130例患者中显效106例，占81．5％；有效17例，占13．1％；无效4例，占3．1％；死亡3例，病死率为2-3％。其死亡原凶为：1例死于双侧多发肋骨骨折、肺挫裂伤、严重肝脾破裂失血性休克；1例死于失血性休克，合并多器官功能不全综合征；1例死于重型颅脑损伤。结论腹部创伤伤情复杂，术前误诊率高，死亡率高，早期诊断、正确处理合并伤和及时手术治疗对挽救患者生命具有重要的临床意义。

Clinical diagnosis and treatment analysis of abdominal trauma on 130 cases patients

Objective To investigate the clinical diagnosis and treatment of abdominal trauma. Methods The clinical data of 130 cases abdominal trauma patients from June 2006 to 2011 were retrospectively analyzed, its cause of injury, injury distribution, injury severity (ISS) score, diagnosis and surgical treatment were analyzed. Results In 130 cases, 106 cases were marked, accounted for 81.5%; 17 cases were effective, accounted for 13.1%; 4 cases were ineffective, accounted for 3.1%; 3 cases were dead, fatality was 2.3%. The cause of death were: one cases died of bilateral multiple rib fractures, pulmonary contusion, severe liver and spleen rupture of hemorrhagic shock; one patient died of hemorrhagic shock, patients with multiple organ dysfunction syndrome; 1 patient died of severe traumatic brain injury. Conclusion Injuries of abdominal trauma are complex, the misdiagnosis rate and mortality is high preoperate, early diagnosis, correct handling of the associated injuries and timely surgery to save the lives of patients has important clinical significance.
