摘    要:目的探讨16层螺旋CT三维成像在骨关节外伤中的应用。方法 65例外伤骨折患者,26例行X线平片检查。65例患者均行螺旋CT扫描及三维重建,在工作站进行容积再现(VR)及多平面重组(MPR),并与CT轴位图像及X线平片相比较。结果 65例外伤性骨折,CT轴位图像显示64例,1例颧弓水平骨折经MPR及3D显示骨折线清晰。26例X线平片,3例肋骨线性骨折或轻微骨裂显示不清;2例脊柱爆裂复合伤骨折片移位、椎管狭窄、多发附件骨折X线平片显示不完全;2例肩胛骨线形骨折,1例第2、4跖骨骨折,1例舟状骨骨折,1例股骨外侧髁骨折,1例肘关节桡骨小头骨折,X线平片显示不清;CT重建后均显示清晰。结论 16层螺旋CT能显示X线平片难以显示的隐匿性骨折,三维重建及MPR成像技术可全面观察骨折的形态变化。

Application on three-dimensional imaging of 16 slice spiral CT in joint injury

Abstract:Objective To discuss the application on three-dimensional imaging of 16 layer spiral CT in joint injury.Methods There were 65 patients with joint injury,and 26 patients were given X-ray examination.65 patients were all given spiral CT and three-dimensional reconstruction,volume rendering(VP) and multiple planar reformation(MPR) were made in workstation,which were compared with CT shaft bit image and X-ray.Results There were 64 patients with CT shaft bit image of 65 patients with joint injury,one patient were diagnose in ophthalmic bow level fractures by MPR and his fracture line was clear from 3D.There were 3 patients’ X-ray of fracture line or minor fractures were unclear.There were 2 patients’ X-ray of spinal bursting fracture combined with fracture displacement,spinal stenosis,multiple attachments fracture showed not completely.There were 2 patients with shoulder blade linear fractures,one patient with fracture in the second and fourth metatarsal bones,one patient with scaphoid fracture,one patient with femoral condyle fractures,one patient with radial head fractures of elbow joint,which X-ray showed unclear,but all showed clear after CT image reconstruction.Conclusion 16 layer spiral CT can show the hidden fractures through X-ray,three-dimensional reconstruction and multiple planar reformation can observe the comprehensive change of fractures.
