社区护理干预对居家糖尿病患者遵医行为的影响

目的 对居家糖尿病患者实施社区护理干预,探讨和分析其对患者遵医行为的影响.方法 将我院收治的167例Ⅱ型糖尿病患者,在患者出院后对其统一建立档案,并按照随机数字表法分为试验组100例和对照组67例.对照组患者行常规的糖尿病知识教育和宣传,以电话随访的方式对患者进行指导;试验组在对照组护理的基础上,护理人员对患者进行家访,给予一对一面对面的社区护理干预指导.结果 与对照组相比,试验组患者在饮食控制、药物治疗、运动治疗以及自我监测、按时进行检查等方面差异有统计学意义（均P＜0.05）.在干预3个月后,试验组患者的空腹血糖、餐后2小时血糖、糖化血红蛋白值明显低于对照组,差异有统计学意义（P＜0.05）.结论 实施社区护理干预,能改善糖尿病患者的遵医行为,提高用药安全,有效控制疾病的进展.

Effect of community nursing intervention on treatment of patients with diabetes

Objective Community nursing intervention was implemented among the diabetes patients at home. The present study was to explore and analyze their impact on the compliant behaviors in patients with diabetes. Methods 167 cases of patients with type 2 diabetes mellitus were divided into the test group （n= 100） and control group （n=67） according to the method of random number table. Routine knowledge education and publicity of diabetes were made among the patients of control group, and the patients were guided by the methods of telephone follow-up. On the basis of control group nursing, family visit was done among the test group by the nurse with the method of one on one, face to face com- munity nursing intervention. Results The diet control, medication, self-monitoring and inspection of the test group were significantly different from that of the control group （P〈0. 05）. The glycosylated hemoglobin of test group was significantly lower after intervention of one month（P〈0.05）. Conclusion Implementation of community nursing inter- vention may improve the compliance behavior of diabetes patients and the drug safety and effectively control the progress of disease.
