原发性肝癌自发性破裂出血的诊断与治疗

目的：总结原发性肝癌自发破裂出血的诊断和治疗经验。方法：17例病人均行手术治疗。9例行单纯肝破裂缝合止血，2例缝合止血并术后行肝动脉插管化疗，1例行缝合并肝动脉结扎出血，5例行肝叶切除术。结果：17例病人的出血术后均得到有效控制。单纯缝合止血者术后半年内死亡，肝叶切除术者存活1年以上。结论：肝癌自发破裂出血的远期疗效取决于早期正确诊断，即刻支持治疗，及时手术止血，适时肝癌切除术后定期随访。

Diagnosis and Treatment of Spontaneous Rupture and Hemorrhage in Primary Hepatoc ellural Carcinoma

Objective:To summarize the diagnosis and treatment of spontaneous rupture and hemorrhage in HCC.Methods:All 17 cases were performed operation:Sutured rupture simply in 9 cases;Sutured and performed hepatic artery embolism chemotherapy after operation in 2 cases;Sutured and ligated hepatic artery in 1 case;Performed hepatoectomy in 5 cases.Results:Hemorrhage of 17 cases stopped after operation.Simple suture group was dead within half years after operation.Hepatoectomy group survived over 1 year.Conclusions:The long term curative effect of rupture and hemorrhage in HCC was decided by early diagnosis,supporting treatment immediately,operation in time,tumor resection timely and follow up after operation.
