外固定架治疗不稳定性桡骨远端骨折

目的探讨外固定架治疗不稳定性桡骨远端骨折28例的术后疗效。方法本组患者共28例29侧;平均年龄59岁。Fernandez分型:Ⅰ型5例5侧、Ⅲ型18例18侧、Ⅴ型5例6侧。术后8周,X线片示骨折愈合,拆除外固定架和科氏针,进行腕关节的康复训练。结果本组28例全部获得随访,随访12～38个月,平均20个月。骨折愈合时间为6～8周,平均7.6周。掌倾角平均9°(-2°～25°),尺偏角平均21°(13°～24°),桡骨短缩4mm的1例,3mm1例,2mm3例,其余均<2mm,采用Gartland和Werley腕功能评分标准:优20例,良6例,可2例,优良率为92.8%。结论外固定架固定是治疗不稳定性桡骨远端骨折的一种理想选择。

Fixation in unstable distal radial fractures with

Objective To explore the clinical effect in 28 cases of unstable distal radial fractures with external fixators. Methods 28 patients with 29 unstable distal radial fractures were treated with close reduction. Their average age was 59 years old. According to Fernandez classification, there were 5case and 5 lateral in typeⅠ, 18 case and 18 lateral in type Ⅲ, 5 case and 6 lateral in type Ⅴ.In the fracture healing proved by X-ray scan, the fixator and pins were removed, and wrist rehabilitation therapy began. Results All the patients involved were followed-up for 12 to 38 months, average 20 months. The bone healing time were 6~8 weeks, average 7.6 weeks. Average palmar tilt 9°（-2°~25°）, radial inclination 21°（13°~24°）. Radial axial shortening, 4 mm in one patient, and 3 mm in 3 patients, and 2 mm in the others. The functional results included 20 excellent cases, 6 good cases, and 2 fair cases according to Gartland-Werley wrist functional assessment.The excellent rate was 92.8%.Conclusion External fixator is a ideal operation to treat unstable distal radial fractures.
