持续正压通气对阻塞性睡眠呼吸暂停综合征心律失常的作用

目的：探讨阻塞性睡眠呼吸暂停综合征（OSAS）严重程度与心律失常间的关系以及经鼻持续气道正压通气（n CPAP）对心律失常的作用。方法：162例OSAS者按呼吸紊乱指数分轻、中、重3组,观察心律失常发生率,比较组间差异性;对16例重度OSAS伴心律失常患者行n CPAP治疗,比较治疗前后监测资料。结果：OSAS组心律失常的发生率明显高于对照组（P〈0.05）,并且随着病情加重而升高（P〈0.05）。n CPAP治疗后,AHI下降,mSaO2上升,平均每小时心律失常发生次数较治疗前显著减少（均P〈0.01）。结论：随着OSAS患者病情加重,心律失常发生率增多,n CPAP能有效降低OSAS患者心律失常发生率。

Effects of CPAP Therapy on Cardiac Arrhythmia in Patients with OSAS

Objective：To investigate the relationship between the severity of OSAS and Cardiac arrhythmia in pa- tients with OSAS （obstructive sleep apnea syndrome） and the effects of n CPAP （ nasal continuous positive airway pressure） therapy on Cardiac arrhythmia. Methods 162patients with OSAS were divided into mild ,moderate and se- vere groups according to the Apnea Hyponea Index, The incidence rate of cardiac arrhythmias, the differences among three groups, And the changes of the incidence rate of cardiac arrhythmias before and after CPAP therapy were observed. Results The incidence of cardiac arrhythmia in patients with OSAS were significantly higher than those in control group （ P〈 0.05） , and increased gradually along with aggravation of the disease condition. After n CPAP therapy, AHI decreased and mSaO2 increased obviously , the number of cardiac arrhythmia per-hour decreased obviously （ P 〈 0.001）. Conclusion With the increasing severity of OSAS, cardiac arrhythmias increased, n CPAP therapy could effectively lower the incidence of cardiac arrhythmias in ptients with OSAS.
