腹膜外乙状结肠造口术临床分析

目的 探讨腹膜外隧道式乙状结肠造口术在直肠癌治疗中的临床效果。方法 对本院自2003～2006年收治的直肠癌施行腹膜外隧道乙状结肠造口术26例患者的资料进行回顾性分析。结果 本组26例，术后造口粘膜出现缺血坏死l例，造瘘口感染l例，造口旁疝2例，其他病例恢复良好。经过3～36个月随访，6～9个月病人排便开始有规律，每天l～2次或隔天1次，每次基本上都能排净，随访病人中约60％病人有便意。结论 腹膜外隧道式乙状结肠造口具有并发症少、排便功能好、易于术后管理等优点，明显提高了患者的生活质量。

Clinical Study on the Treatment of the Extraperitoneal Sigmoidostomy

Objective To explore the clinical effects for extra peritoneal colostomy in treatment of anorectum carcinoma. Methods There were 26 patients with rectal carcinoma received extraperitoneal colostomy from 2003 to 2006 in our hospital. Clinical data of 26 patients followed up for 1～3 years were retrospectively analyzed. Results There were 26 patients in our group. The hitter＇s complications included 1 case of sigmoidostomic catarrh necrosis, 1 case of skin inflammation around the colostomy, 2 cases of the side-colostomy bemia. The other patients recovered better. The patients with extraperitoneal colostomy were followed up for 3～ 36 months. After 6 ～9 months, they began to defecate regularly. They defecated 1～ 2 times every day or the following day, and they defecated clearly every time. 60 percent of the patients got continence colostomy. Conclusion The extrapertitoneal sigmoidostomy has less complication, better functions of defecation and easier management after operation. Then it could improve the life quality of the patients.
