吸入布地奈德治疗慢性阻塞性肺疾病急性加重期的疗效研究

目的观察雾化吸入布地奈德混悬液治疗慢性阻塞性肺疾病（COPD）急性加重期患者的临床疗效.方法 40例COPD急性加重期住院患者随机分为两组：治疗组雾化吸入布地奈德2 mg,2次/d.对照组静脉滴注糖皮质激素.两组患者同时还接受吸氧、抗感染及支气管扩张剂等常规治疗.治疗前、治疗后1 h、3 d、5 d、7 d分别检测肺功能1秒钟用力呼气容积（FEV1）、动脉血氧分压（PaO2）和二氧化碳分压（PaCO2）.结果两组COPD患者治疗7 d后与治疗前比较FEV1、PaO2及PaCO2的均值间差别均有显著性意义（P＜0.05）.在各观察时间点,两组患者的FEV1、PaO2及PaCO2的均值间差别无显著性意义（P＞0.05）.结论雾化吸入布地奈德能改善COPD急性加重期患者的肺功能与临床症状.

The Effect of Inhaled Budesonide in Persons with Acute Exacerbations of Chronic Obstructive Pulmonary Disease

Objective To evaluate the therapeutic effect of inhaled budesonide in persons with acute exacerbations of chronic obstructive pulmonary disease(COPD).Methods 40 patients with moderate and severe COPD were randomly divided into two groups: the treatment group and the control group.The treatment group took inhaled budesonide 2mg,twice/day and the routine treatment.The control group took systemic glucocorticoids and the routine treatment.FEV_1,PaO_2 and PaCO_2 were measured at before and 1 hour,3 days,5 days and 7 days after the treatment.Results 40 patients finished the study.Compared with before the treatment,the two groups showed significant improvement in FEV_1,PaO_2 and PaCO_2(P<0.05)after receiving 7 days of inhaled or systemic corticosteroids.Compared with the control group,there were not much difference in FEV_1,PaO_2 and PaCO_2 between the treatment group and the control group(P>0.05).Conclusion The study suggested that the therapy of inhaled corticosteroids(ICS)can improve pulmonary function and clinical symptoms in patients with acute exacerbations of COPD.
