留置导尿患者集尿袋更换时间的Meta分析

目的探讨留置导尿患者更换集尿袋的最佳时间。方法全面检索国内相关文献,采用Cochrane系统评价方法对符合纳入、排除标准的随机对照试验进行Meta分析,研究对象为已发表的有关留置导尿患者集尿袋更换的临床对照试验,观察指标为留置导尿患者集尿袋不同更换频率(每天更换、每3d更换及每周更换)下的尿培养阳性率,干预有效性的效应量采用相对危险度(odds ratio,OR)。结果共检索到10篇合格文献。Meta分析结果提示:留置导尿患者集尿袋每天更换与每3d更换、每3d更换与每周更换、每天更换与每周更换发生泌尿系感染的OR值分别为2.3195%CI(1.45,3.68),P=0.0004]、0.5495%CI(0.32,0.92),P=0.02]、1.3395%CI(0.91,1.95),P=0.14]。结论留置导尿患者若每3d更换1次集尿袋,发生泌尿系感染的风险最低。

A Meta Analysis of Replacement Time of Urine Drainage Bags in Patients Undergoing Urinal Indwelling Catheterization

Objective To investigate the maximal time for replacing the urine drainage bags of patients undergoing urinal indwelling catheterization.MethodsRelated domestic literature were searched comprehensively.Using Cochrane system review,the randomized controlled trials meeting the inclusion and exclusion criteria were processed by Meta-analysis.The subjects referred to the published clinical control trials concerning replacing the urine drainage bag of patients undergoing urinal indwelling catheterization.The observing indicators involved the positive rates of urine culture in accordance with the replacement frequencies（daily,every 3 days and weekly） of the urine drainage bags of patients undergoing urinal indwelling catheterization.Odds ratio（OR） was used as the effect size of the intervention effects.Results10 qualified literature were retrieved.The results of the Meta analysis suggested that the results of replacing the urine drainage bags daily,every three days and weekly wereOR=2.31,95%CI（1.45,3.68）,P=0.0004;OR=0.54,95%CI（0.32,0.92）,P=0.02;OR=1.33,95%CI（0.91,1.95）,P=0.14].ConclusionIt is appropriate to replace the drainage bags every three days compared with the other two replacement time interval.
