酚妥拉明、多巴胺治疗新生儿肺炎并发心衰

21例新生儿肺炎并发心衰,经西地兰等综合治疗无效,改用酚妥拉明1mg/kg。多巴胺0.5mg/kg、10％GS30ml缓慢静脉滴注,20例于48小时内心衰症状、体征改善,仅1例并存先心病,用药持续7天心衰才得以纠正。本组酚妥拉明、多巴胺平均用量分别为1.3mg/kg/日、0.65mg/kg/日,平均用药天数3.2天。本文讨论了酚妥拉明、多巴胺治疗心衰的机理、疗效、用法及副作用。

Treatmentf of the heart failure due to neonatal pneumonia by phentolamine and dopamine

It was ineffect that use of comprehensive treatment of cedilanid etc for 21 patients with heart failure due to neonatal pneumonia, but intravenous dripping slowly a mixture of 10％ glucose 30ml、phentolamine (1.0mg/kg), and dopamine (0.5mg/kg) improved symptoms and sings of heart failure of 20 cases within 48 hours, only one congenital heart disease was not improved untill 7 days after. The average with dose of the patients of this group for phentolamine and dopamine was, respectively, 0.3mg/kg and 0.65mg/kg daily. The mean time of medication was 3.2 days. The authors discussed the mechanism, therapeutic effect, administration and side effect of phentolamine and dopamine on treatment of the heart failure.
