应用Amplatzer封堵器经皮介入治疗膜周部室间隔缺损技术分析

目的分析总结膜部室间隔缺损介入封堵治疗的技术参数和要领。方法总结近三年来27例室间隔缺损的介入封堵技术资料,分析室缺大小与封堵器大小选择的关系;室缺类型、缺损大小对手术操作的影响以及不同的超声手段对手术支持的必要性等。结果室缺大小与封堵器大小的选择有以下线性关系:y=1.2x+1.5;囊袋型室缺成功率相对较低为86%,手术用时最长为136.7±32.7分钟,与漏斗型用时94.8±15.0分钟相比差异有显著性;经食道超声手术用时延长,达133.9±34.7分钟,经胸超声平均只需100.7±27.6,可以满足手术要求。结论根据室缺大小与封堵器大小的选择有良好的线性关系;囊袋型出口手术费时长,成功率偏低;多数可使用经胸超声支持进行介入封堵。

Technical analysis of interventional occlusion for patients with membraneous ventricular septal defection.

Objective To analyze and conclude the main technic factors of interventional occlusion for patients with congenital membraneous ventricular septal defection(VSD). Methods Data of 27 cases of VSD patients who accepted interventional occlusion had been analyzed. Then we inferred the linal relationship between the VSD size and occluder size. The success rate of the interventional operation affected by different VSD types and sizes and the role of ultrosound cardiogram in the operation also was assessed. Results Selection of size of VSD occluder had a good linal relationship with VSD size which can be expressed as y=1.2x 1.5; The sac type of VSD had significant affection to the success rate of the operations. Conclusion The size of occluder selection has good a linal relationship with the VSD size; The operation of sac type VSD last longer time and has relative lower success rate.
