肿瘤所致急性喉梗阻的处理

目的探讨肿瘤所致急性喉梗阻的有效诊断、救治措施。方法对1987年3月~2003年2月接诊的48例肿瘤急性喉梗阻的患者资料进行总结,依据肿瘤特点及患者情况,采取了有效的检查措施,分别实施了紧急气管切开、限期气管切开和气管插管术。结果11例行紧急气管切开,23例行限期气管切开,14例行气管插管,全部病例喉梗阻解除,抢救成功率100%;6例一期行原发肿瘤切除,42例行二期手术切除原发灶。结论对于喉梗阻患者急诊医生应与耳鼻喉科医生联合进行诊治;肿瘤所致喉梗阻应首先建立通畅的通气道,然后再考虑原发病的治疗。

The tveatment of laryngeal obstruction caused by tumor

Objective To explore the therapy of laryngeal obstruction derived from tumor.Methods Forty eight cases with laryngeal obstruction derived from tumor were reviewed.According to the tumor characteristic and patient condition,measure was taken and urgent tracheotomy,confining time tracheotomy and tracheal intubation were performed.Results Eleven cases were treated with urgent tracheotomy,23 cases with confining time tracheotomy,14 cases with tracheal intubation.All patients were free from laryngeal obstruction,the urgent treatment success rate was 100%.The original tumor was removed at the same time for 6 cases,and for the other 42 cases,tumor was resected second time.Conclusion To the laryngeal obstruction patients,the emergency doctor should make the diagnosis and give treatment with the aid of otolaryngologist.For the laryngeal obstruction derived from tumor,air passage should be firstly built,and then consider the original disease treatment.
