摘 要: | 【目的】探讨后交叉韧带(PCL)断裂对外侧副韧带(LCL)腓骨止点生物力学的影响。【方法】选择新鲜尸体正常成人膝关节标本12侧作为PCL完整组,在200N栽荷下,测试膝关节0°、30°、60°、90°外侧副韧带腓骨止点处应变,测试完毕后随机将标本遣模成前外侧束(ALB)断裂组和后内侧束(PMB)断裂组,各6侧,在上述条件下测试,再将12侧标本的PCL中下1/3全部切断作为全断组进行测试。【结果】膝0°位时,LCL腓骨止点各组均为压应变,差异无显著性(P〉0.05);膝30°位时,各组均为压应变,PMB断裂组较完整组及全断组增大,差异有显著性(P〈0.05);膝60°位时,完整组和PMB断裂组为压应变,全断组和ALB断裂组为拉伸应力。全断组和ALB断裂组与完整组和PMB断裂组比较,差异有显著性(P〈0.05),但全断组和ALB断裂组及完整组和PMB断裂组,组间两者比较差异无显著性(P〉0.05);膝90°位时,各组均为拉伸应变,全断组和ALB断裂组应变大于完整组和PMB断裂组,差异有显著性(P〈0.05)。但全断组和ALB断裂组及完整组和PMB断裂组,组间两者比较无显著性差异(P〉0.05)。【结论】膝0°、30°位时,PCL断裂对LCL腓骨止点生物力学无明显影响,膝60°、90°位时,PCL中PMB断裂对LCL腓骨止点生物力学无明显影响;膝60°、90°位时,PCL断裂或其中的ALB断裂时均对LCL腓骨止点生物力学产生明显的影响。

Abstract: | [Objective]To investigate the effect of the posterior cruciate ligament(PCL) rupture on the biomechanics ot tibula attachment of lateral collateral ligament(LCL). [MethodslThe specimens in this experiment were 12 normal fresh knee joints of adult male. At first, all the 12 specimens, whose lateral collateral ligament at fibula attachment straining were measured at different angles (0°,30°,60°and 90°) under 200N , were regarded as the group with intact PCL. Then, 6 specimens were randomly treated as the group with the anterolateral bundle of PCL ruptured. The other 6 knees were treated as the group with posteromedial bundle of PCL ruptured. Firstly, the straining of lateral collateral ligament of the two groups was measured. At last, the PCL was cut off completely in all 12 specimens in the group with the PCL completely ruptured and the measurement at the four angles under the four loads similarly was done. [Results] Straining point at knee angle 0°: The strains of lateral collateral ligament at fibula attachment in all groups were compressive. They were no significant difference ( P〉0.05).Straining point at knee angle 30°: The strains of lateral collateral ligament at fibula attachment of all groups were compressive. The strains in the intact PCL group were bigger than those in the intact PCL group and the complete ruptured group ( P〈0. 05). Straining point at knee angle 60° the strains of the intact PCL group and the PMB ruptured group were compressive. While the strains of the ALB ruptured group and the complete ruptured group were tensile. The differences among the intact PCL group, the PMB ruptured group, the ALB ruptured group and the complete ruptured group were significance( P 〈0.05). While the difference between the ALB ruptured group and the complete ruptured group was no significance ( P 〉0.05).
