长柄人工股骨头置换治疗高龄股骨转子间骨折的疗效分析

目的探讨长柄骨水泥型双极人工股骨头置换治疗高龄不稳定性股骨转子间骨折的临床疗效。方法对2005-01-2010-06采用长柄骨水泥型双极人工股骨头置换治疗75岁以上不稳定性股骨转子间骨折62例，平均随访15个月，观察术后并发症、骨折愈合时间、关节功能情况。结果术后53例患者获得随访，无一例死亡；有1例出现下肢深静脉栓塞，经治疗后痊愈出院；无褥疮、肺部感染、髋关节脱位等常见并发症发生。根据Harris评定标准评价：优43例（81．1％），良7例（13．2％），可3例（5．6％），差0例，优良率达94．3％。结论长柄骨水泥型双极人工股骨头置换治疗高龄不稳定性股骨转子间骨折具有创伤小、手术时间短、下地活动早、并发症少、髋关节功能恢复好等优点，是一种较理想的治疗方法。

Long stem femoral head prosthetic replacement in the treatment of elderly patients with femoral intertrochanteric fracture

Objective To explore the clinieal effect of long stem cemented bipolar femoral head prosthetic replacement in the treatment of elderly patients with unstable femoral intertroehanterie fracture. Methods Sixty-two patients aged above 75 years old who suffered from unstable femoral intertrochanterie fractures were treated by long stem cemented bipolar femoral head prosthetic replacement from January 2005 to July 2010. The average follow-up time was 15 months, the postoperative complications, the fracture healing time and joint function were observed. Re-suits There was no death case in 53 patients who were followed up. Deep venous embolism of lower limbs was found in one patient, who was cured and discharged after treatment. No complicationts, such as bad sore, pulmonary infec-tion, hip dislocation and so on were found. According to the standard of harris, excellent effect was found in 43 pa- tients （81.1%）, good in 7 pationts （ 13.2% ）, fair in 3 patients （5.6%）. The excellent and good rate was 94. 3%. Conclusion The long stem cemented bipolar femoral head prosttcetic replacement in the treatment of elderly patient with unstable femoral intertrochanteric fracture has advantages of less trauma, shorter operative time, earlier ambulation, less complications, better recorery of hip function and so on. and it is an more ideal treatment method.
