老年非酒精性脂肪性肝病的临床特征

目的分析老年非酒精性脂肪性肝病(NAFLD)的临床表现、实验室检查、超声及合并疾病等特征。方法对临床诊断NAFLD的128例患者临床资料进行回顾性分析,比较48例老年(≥60岁)患者和80例中青年患者的一般资料、临床表现、血常规、血糖、血脂、肝功能化验、超声及合并疾病等特征。结果确诊NAFLD时,老年组患者的女性比率高于中青年组(分别为60.4%和26.3%,P<0.05),临床表现相似,但空腹血糖、总胆固醇和低密度脂蛋白胆固醇水平较高,白蛋白水平较低,合并高血压、2型糖尿病等代谢综合征相关疾病的比率高于中青年组(分别为85.4%和36.3%,P<0.01)。结论和中青年患者比较,老年NAFLD的女性比率更高,更常合并高血压、2型糖尿病等疾病。

Clinical features of elderly patients with non-alcoholic fatty liver disease

Objective To evaluate clinical features of elderly patients with non-alcoholic fatty liver disease（NAFLD）.Methods Clinical data of 128 patients diagnosed with NAFLD was reviewed.Elderly patients（≥60 years） were compared with younger patients（60 years） in terms of clinical manifestation,blood routine test,biochemical,ultrasonographical features and incidence of combined diseases.Results Among128 patients,the initial diagnosis of NAFLD was made in 48 patients at an age ≥60.The female/male ratio was higher in elderly patients compared with younger patients（29/48 vs 21/80,P0.05）.The elderly patients had similar clinical manifestations,but higher fasting blood glucose,total cholesterol and low density lipoprotein cholesterol levels,and lower albumin levels compared with younger patients.The incidence of combined diseases such as hypertension and/or diabetes mellitus type Ⅱ was much higher in the elderly than in younger group（85.4% vs 36.3%,P0.01）.Conclusion Elderly patients with NAFLD had similar clinical manifestations,but higher female/male ratio and incidence of combined hypertension and/or diabetes mellitus type Ⅱ compared with younger patients.
