CDx2和Hep在胃癌组织中的表达及临床意义

目的 探讨CDx2和Hep在胃癌组织中的表达及临床意义。方法 应用免疫组织化学EnVision二步法检测9例正常胃黏膜、15例小肠黏膜、8例结肠黏膜、15例胃黏膜肠化生和130例胃癌组织中CDx2和Hep的表达。结果 正常胃黏膜、小肠黏膜、结肠黏膜、胃黏膜肠化生和胃癌组织中CDx2阳性表达率分别为0、80％（12／15）、87．5％（7／8）、86．7％（13／15）、37．7％（49／130）；Hep阳性表达率分别为11．1％（1／9）、86．7％（13／15）、12．5％（1／8）、100％（15／15）、55．4％（72／130）。肠型胃癌组CDx2和Hep阳性表达率嘎显高于弥漫型胃癌组（P〈0．05），CDx2阳性表达率与性别、淋巴结转移情况、TNM分期相关（P〈0．05）；Hep阳性表达率与浸润深度和胃癌Lauren分型相关（P〈0．05）。结论 CDx2和Hep在鉴别胃癌组织学分型和预测临床预后方面有一定价值。

Expression and clinical significance of CDx2 and Hep in gastric carcinoma

Objective To investigate the expression and clinical significance of CDx2 and hepatocyte antigen(Hep) in gastric carcinoma.Methods The expression of CDx2 and Hep in 9 cases of nomal gastric mucosa,15 cases of small intestinal mucosa,8 cases of colon mucosa,15 cases of intestinal metaplasia of the stomach and 130 cases of gastric carcinoma were detected with immuohistochemical EnVisionTM Systems measures.Results The positive rates of CDx2 expressed in the nomal gastric mucosa were 0,small intestinal mucosa 80%(12/15),colon mucosa 87.5%(7/8),intestinal metaplasia of the stomach 86.7%(13/15),gastric carcinoma 37.7%(49/130);The positive rate of Hep was 11.1%(1/9),86.7%(13/15),12.5%(1/8),100%(15/15),55.4%(72/130) respectively.There was much higher positive expression of CDx2 in intestinal-type carcinoma than in diffuse-type gastric carcinoma(P<0.05);The positive expression of CDx2 was found to correlate with gender,lymph node metastasis and TNM stages(P<0.05).There was a correlation between Hep expression and wall invasion,gastric carcinoma Lauren classification(P<0.05).Conclusion These data suggests that CDx2 and Hep have significant value in distiguishing hisyologic type and in predicting the prognosis of gatric carcinoma.
