帕夫林联合确炎舒松治疗口腔扁平苔藓的初步观察

目的　观察帕夫林联合确炎舒松局部注射治疗口腔扁平苔藓 (OLP)的临床疗效 ,了解帕夫林对OLP患者治疗前后T淋巴细胞亚群的影响。方法　 5 0例OLP患者随机分成两组 ,每组 2 5例。A组 :确炎舒松混悬液病损区局部注射每周 1次 ,共 4次 ,加用帕夫林口服每次 2片 ,每天 2次 ,共 2个月 ;B组 :确炎舒松混悬液病损区局部注射每周 1次 ,共 4次。结果　治疗开始后 6个月检查疗效 ,结果A组有效率 88% ,B组有效率 6 8% ,经统计学检验 ,A组与B组有显著性差异 (P <0 .0 5 )。OLP患者治疗前外周血CD3、CD8比率高于正常人 (P <0 .0 5 ) ,CD4 /CD8低于正常人 (P <0 .0 5 ) ,加用帕夫林治疗后CD3、CD8和CD4 /CD8比率恢复正常。结论　帕夫林与确炎舒松联合应用疗效高于单纯确炎舒松局部注射治疗组 ,其作用机理可能与帕夫林调节患者的T淋巴细胞功能有关

The Effect of TGP Combined with Topical Application of Triamcinolone on Patients with Oral Lichen Planus

Objective To explore effect of total glucosides of paenia (TGP) combined with topical application of triamcinolone on patients with oral lichen planus (OLP). Methods 50 cases of OLP were divided into two groups randomly: Group A (25 cases) were treated with triamcinolone topically once a week for one month, and were given TGP 2 tablets twice 1 day, 2 months totally. Group B (25 cases) were given triamcinolone topically only (the same with group A). Results CD3 and CD8 ratio of patients with OLP increased significantly and CD4/CD8 ratio decreased significantly compared with that of health people (P<0.05) before treatment. In group A, 22/ 25 (88 per cent) of patients showed an improvement in symptoms in 6 months after treatment. CD3, CD8 and CD4/CD8 ratio of patients were recovered to normal after treatment. In group B, 17/25 (68 per cent) of patients showed an improvement in symptoms in 6 months after treatment. Conclusion TGP combined with topical application of triamcinolone is effective for the treatment of OLP. TGP may act through biphasical regulation effects on the immunological balance of the patients.
