原发性脾脏肿瘤的诊治

目的:探讨原发性脾脏肿瘤的诊断和治疗。方法:对16例原发性脾脏肿瘤患者的临床表现、影像学特点及外科治疗方法进行回顾分析。结果:脾脏原发性肿瘤临床表现无特异性,影像学检查能早期发现病变。B超、CT术前定位诊断正确率为100%。结论:影像学检查是原发性脾脏肿瘤诊断的主要手段。根治性脾切除(必要时合并胰体尾切除)是治疗原发性脾脏恶性肿瘤的主要手段,早期诊断、早期治疗是影响本病预后的关键因素。

Diagnosis and treatment of primary splenic neoplasmas

Objective To investigate characteristic of primary splenic neoplasms(PSN) and surgical therapeutics. Method The clinical data, characteristics of image, methods of surgical treatment about 16 cases of PSN were analyzed retrospectively. Results The clinical manifestations of PSN was not specific. Recent advances in imaging techniques had allowed preoperative detection of splenic tumors during a screening for splenomegaly. Ultrasonography, CT scan were performed in all cases. Conclusions For malignant splenic tumor, radical splenectomy and additional distal pancreatectomy should be done if neccessary. Early diagnosis and treatment is more important for prognosis of PNS.
