CT导向下经皮穿刺肺活检对肺真菌的诊断价值

目的:探讨CT导向下经皮穿刺活检对肺真菌的诊断价值。材料与方法:回顾性分析26例经皮肺穿刺活检病理证实的肺真菌病的病例资料,所有病例均行CT检查,其中23例平扫,3例直接增强,26例行细针抽吸及切割活检送病理组织学检查。结果:CT显示单发肺结节7例,多发肺结节11例,结节伴斑片状影5例,3例呈实变样表现,26例经细针抽吸活检3例确诊曲菌,3例可疑真菌,26例切割活检病例均确诊为真菌,其中曲菌11例,隐球菌11例,马内菲青霉菌1例,毛霉菌1例,球孢子菌1例,未分类真菌1例,发生少量气胸3例,少许出血2例。结论:对于肺部难以确诊的肺真菌病例,CT导向下经皮穿刺切割活检是一种安全有效的方法。

The Diagnostic Value of CT-Guided Percutancous Lung Biopsy in Pulmonary Fungus

Purpose：To detect the diagnostic value of CT-guided percutancous lung biopsy in pulmonary fungus. Materials and Methods：The pulmonary fungus of 26 cases identified by pathology with CT-guided percutancous needle biopsy were retrospective analyzed,26 cases were exam by CT,23 cases were plan scanning,3 cases were enhanced.The needle aspiration and cutting in 26 cases by CT-guided percutancous lung biopsy,the specimen were exam by pathology. Results：The CT findings of pulmonary fungus were divided into 4 types：（1）solitary mass or nodule in 7 cases;（2） multi-focal mass or nodule in 11 cases;（3）nodule and patchy shadow in 5 cases;（4）patchy consolidation in 3 cases with air bronch-gram.Pulmonary fungus were suspiciously diagnosed and diagnosed in 6 cases by needle aspiration biopsy,26 cases were diagnosed by needle cutting biopsy,including aspergillus 11 cases,cryptococcosis 11 cases,penicillum marnefei 1 case,mucormycosis 1 case,coccidioidomycosis 1 case,no classifying fungi 1 case.There is pneumothorax in 3 cases,litter hemorrhage in 2 cases. Conclusion：It is a safe and effective means by CT-guided percutancous lung biopsy in diagnosis of the pulmonary fungus.
