螺旋CT在口腔颌面部创伤中临床应用价值的研究

摘    要:目的:探讨螺旋CT三维成像和多平面成像技术在颌面部创伤中的应用价值。材料与方法:对62例颌面部骨折患者行薄层连续扫描后,利用工作站进行图像后处理,选用多种软件技术,多平面重组(MPR)、三维表面遮盖显示(3D-SSD)等重组观察,重建颌面部骨折的三维影像,获得最佳图像。结果:螺旋CT MPR成像清晰显示了所有的颌面部骨折的部位、类型、异物的数目及碎骨片的移位及周围软组织的损伤情况,特别对细微骨折及深部骨折显示优于3D-SSD。而3D-SSD属立体成像,可直观展现骨折的部位,骨折线的走行,骨质的碎裂程度及移位情况。结论:多层螺旋CT MPR结合3D-SSD成像能够明确诊断颌面部骨折,可直观地观察到颌面部骨折在三维立体空间的实际大小、形态位置及周围结构的解剖关系,并为其临床治疗提供依据,对颌面部骨折术前制定手术方案和术前、术后对比均有重要的指导作用。

Abstract:Purpose:To evaluate spiral CT three-dimensional imaging and multi-planar imaging in maxillofacial trauma application. Materials and Methods:62 patients with maxillofacial trauma patients with thin continuous scanning,the use of image post-processing workstation,using a variety of software technologies,multi-planar reformation(MPR),surface shaded display(3D-SSD)and other re-observation,reconstruction three-dimensional images of maxillofacial fractures,get the best image. Results:The spiral CT MPR images clearly show all parts of maxillofacial fractures,type,number of foreign bodies and bone fragments and pieces of the displacement damage of surrounding soft tissue,especially the subtle fractures and is superior to the deep fractures 3D-SSD.The 3D-SSD is a three-dimensional imaging,can directly show the fracture site,fracture line courses,the degree of bone fragmentation and shifting. Conclusion:The multi-slice spiral CT MPR images combined with 3D-SSD can be diagnosed fractures,can be visually observed maxillofacial fractures in the actual three-dimensional space,size,shape location and anatomic relationship around,and their clinical treatment the basis of preoperative maxillofacial fractures surgical planning and preoperative and postoperative contrast are important guiding role.
