多层螺旋CT图像后处理技术在骨关节创伤中的应用价值

摘    要:目的探讨多层螺旋CT图像后处理技术在骨关节创伤中的应用价值。方法60例骨关节创伤患者行多层螺旋CT薄层扫描后,在工作站进行多平面重建(MPR)、三维重建(包括SSD法和VRT法),分析各种方法对骨折的显示,并与平片对照。结果X线平片对骨折的检出率为70.8%,MSCT横断位、MPR、SSD、VRT图像对骨折的检出率分别为98%、98%、90%和99%。MSCT横断位可发现细小骨折。但缺乏立体信息,三雏重建图像(SSD、vRT)能良好的显示骨折部位骨结构的空间关系,SSD立体感最强,MPR显示骨折线走行和移位程度更有优势且能兼顾软组织的改变。结论多层螺旋CT横断位结合MPR、VRT、SSD可提高骨关节创伤的诊断准确性,简化思维过程,为临床诊治提供更多有价值的信息。

Abstract:Objective] To discuss the diagnostic value of multislice CT post-imaging processing, technique on trauma of bones and joints. Methods] After 60 patients with trauma of bones and joints were underwent thin-slice volume scan by MSCT, performed reconstruction in ways of multi-planer reconstruction(MPR), 3D reconstruction(surface shaded display SSD and volume rendering VR) at work-station, then compared with X-ray plain film. Results] 60 patients with fracture or dislocation, analyzed with axial images of MSCT, MPR and VR. Fracture was detected totally in 89 locations. The fracture detection rate of axial images of MSCT is 98%, MRP is 98%, VR is 99%, SSD is 90%, X-ray plain film is 70.8%. Axial images of MSCT could show tiny fracture, and 3D image had more advantages in displaying space relationship of fracture and dislocation. MPR was better in displaying courses of fracture line. Conclusion] Axial imaging MSCT, combined with MSCT imaging postprocessing technique can increase the diagnostic accuracy, simplify the course of diagnosis and give more help to clinical treatment.
