经产妇的分娩特点及新生儿情况分析

目的　了解经产妇的分娩特点及妊娠结局。方法　对 5 2 9例经产妇 (研究组 )与 2 2 1例初产妇 (对照组 )的分娩方式、产程、并发症及新生儿情况等进行分析比较 ,并观察末次妊娠距本次分娩时间对产程的影响。结果　 (1)两组产妇产程间的差别有显著性意义 (P <0 0 0 1)。 (2 )两组中巨大胎儿、胎儿窘迫、新生儿窒息及死亡的发生率间差别有非常显著性意义 (均P <0 0 0 1)。 (3)本次分娩距前次分娩的时间与产程间有一定的线性依从关系 (P<0 0 0 0 1)。 (4 )两组产妇剖宫产原因的构成比间差别有非常显著性意义 (P <0 0 0 1)。结论　经产妇的产程明显比初产妇短 ,但随着本次分娩距前次分娩时间的延长 ,产程也逐渐延长。产科医生应加强对经产妇的产前保健 ,积极预防巨大胎儿的发生 ,以减少产程中的异常情况

Analysis on the behavior of labor and outcome of newborn of multipara

Objective To comprehend the behavior of labor and outcome of multipara.Methods The mod,stages,complications of labor and outcome of newborn of 529 cases of multipara and 221 cases of primipara were analysed.The effect of the time of the last pregnancy to this pregnancy on the stages of labor were studied.Results (1)The time of CCN in two groups showed significant difference(P<0.001).(2)The incidences of fetal macrosomia,fetal distress,asphyxia of newborn and mortality rate of newborn showed much significant differences(P<0.001).(3)The time of stages of labor of multipara was correlated with the time of the last pregnancy to this pregnancy(r=0.297,P=0.000).(4)The constituent ratio of causes of cesarean section in two groups showed much significant difference (P<0.001).Conclusion The time of stages of labor of multipara is shorter obviously than that of primipara.The longer time of the last pregnancy to this pregnancy is,the longer the time of stages of labor is.Obstetricians should take more care of multipara.
