不同弹力绷带联合包扎治疗在乳腺癌相关淋巴水肿患者中的应用效果

目的探讨不同弹力绷带联合包扎治疗在乳腺癌相关淋巴水肿患者中的应用效果。方法选取2018年5—12月淋巴水肿护理门诊收治的38例乳腺癌相关淋巴水肿患者,采用泡沫块绷带+低弹力绷带+高弹力绷带联合包扎,实施综合淋巴消肿治疗。治疗前1 d,治疗后10、20 d测量患侧上肢周径。对比治疗前1 d和治疗20 d后患者肢体周径比率、组织水分比率、皮肤纤维化发生率。结果治疗10、20 d后,患侧上肢5个测量点周径均小于治疗前,差异均有统计学意义(P<0.05)。治疗20 d后患者的患侧上肢肢体周径比率、组织水分比率、皮肤纤维化发生率均较治疗前降低,差异有统计学意义(P<0.05)。结论用不同弹力绷带联合包扎实施综合淋巴消肿治疗可有效地改善乳腺癌患者患侧上肢的淋巴水肿程度,缩短疗程,同时可改善淋巴水肿皮肤纤维化程度,值得临床推广。

Effects of different elastic bandages dressing on breast cancer associated lymphedema

Objective To explore the effects of different elastic bandages dressing on breast cancer associated lymphoedema.Methods From May to December 2018,38 patients with breast cancer associated lymphoedema admitted to lymphoedema nursing outpatient were treated with complex decongestion therapy(CDT),which is a combined dressing with foam block bandage,low elastic bandage and high elasticity bandage.The perimeter of the affected limb were measured 1 day before treatment,10 and 20 days after treatment.The limb perimeter difference ratio,tissue water ratio and skin fibrosis rate were compared between 1 day before treatment and 20 days after treatment.Results After 10 and 20 days of treatment,the perimeters were all lower than those before treatment at 5 measuring points,and the differences were statistically significant(P<0.05).The limb perimeter difference ratio,tissue water ratio and skin fibrosis rate 20 days after treatment were all lower than before treatment,and the differences were statistically significant(P<0.05).Conclusions CDT treatment with different elastic bandages can effectively reduce the degree of breast cancer associated lymphoedema,shorten the course of treatment,and reduce the degree of lymphoedema skin fibrosis,which is worthy of clinical promotion.
