无气腹腹腔镜与腹腔镜子宫肌瘤剔除术对机体免疫功能影响的研究

摘    要:目的探讨无气腹腹腔镜(GLO)与腹腔镜子宫肌瘤剔除术(LO)对机体免疫功能的影响.方法选择采用无气腹腹腔镜行子宫肌瘤剔除的病人19例和腹腔镜子宫肌瘤剔除的患者21例,测定术前及术后1、4d外周血免疫球蛋白IgG及补体C_3、C_4水平以及T淋巴细胞亚群的表达水平,结果GLO组术后1d、4d IgG、C_3、C_4水平无明显改变(均P>0.05),LO组术后1d IgG、C_3较术前有显著下降(P<0.05);GLO组术后1d仅CD_3~+较术前下降(P<0.05),LO组术后1d、CD_3~+、CD_4~+、CO_(?)~+均有显著下降(P<0.05),且两组比较差异有显著性(P<0.05).结论无气腹腹腔镜子宫肌瘤别除术对机体免疫功能的损伤较腹腔镜肌瘤剔除术小,有利于患者恢复.

Abstract:objective To study the effect of gasless laparoscopic operation (GLO) and laparoscopic operation(LO) on immune function in patients with uterine myomas.Methods A total of 19 patients with uterine myomas received GLO,21 patients were through LO IgG,compiement 3 (C_3),complement 4(C_4),and T lymphocyte Subpopulation were measured at before 1 and 4 day after oparation respectively.Results Ther was on significance in IgG,complement 3 and complent4 in the GLO Group on before and 1 and 4d after operation (P>0.05),The IgG and complement 3 levels decreased on the 1d in the LO Group (P>0.05);The levels of CD_3~ in the GLO Group decreased significantly on the 1 postoperative day(P<0.05),then the levels of CD_3~ and CD_8~ in the LO Group decreased significantly (P<0.05) CD_3~ ,CD_4~ ,and CD_8~ levels were significantly loner in the LO Group the in the GLO Group on the 1 and 3 postoperative day.Conclusions Gasless laparoscopic operation does not significantly affect immune function compared with laparoscopic because of less injury,while will be advantageous for a rapid recovery.
