康复专科护理规范化流程管理在腰椎间盘突出症患者保守治疗中的效果观察

目的 观察康复专科护理规范化流程在腰椎间盘突出症患者保守治疗中的应用效果.方法 回顾性分析我科198例接受保守治疗的腰椎间盘突出症患者,按时间段分为对照组和观察组,对照组2013年7月至12月收治的92例患者采用传统常规护理方法,观察组2014年1月至6月收治的106例患者在常规护理基础上规范康复专科护理流程:根据患者病情与发展阶段制定并按顺序实施康复专科护理措施,即急性水肿期的卧床休息及心理护理、疼痛护理、正确卧位姿势训练与上下床转移训练;疼痛缓解期的腰椎牵引护理、腰围护理、中医透药、针灸理疗护理;恢复期腰背肌功能锻炼、脊柱保健操文体康复,以及出院前的健康指导等康复专科护理方法.观察两组患者疾病症状改善时间、总疗程、总有效率、1年复发率以及不良反应等.结果 观察组患者临床症状改善时间、总疗程及治疗效果明显优于对照组(P<0.05),随访1年复发率也明显低于对照组(P<0.05),临床治疗过程中两组均未发生明显不良反应.结论 规范化的康复专科护理流程管理可以快速有效缓解腰椎间盘突出症患者的症状,提高治愈率,缩短康复时间,减少复发率,可明显改善腰椎间盘突出症患者的生活质量和活动能力.

Observation about effect of standard procedure management of the recovery depart-ment in conservative treatment of lumbar disc herniation patients

Objective To observe the effect of standard procedure management of the recovery department in conservative treatment of lumbar disc herniation patients. Methods Retrospective analysis of 198 cases of conservative treatment inpatients with lumbar disc herniation, all the patients were divided into a control group and an observation group according to time buckets. 92 patients in the control group which received treatment from July 2013 to December 2013 were taken care of according to conventional nursing method; 106 patients in the observation group which were subject to treatment over the period from January 2014 to June 2014 received standard recovery nursing on basis of origi-nal conventional nursing. For the observation group, the hospital adopted professional nursing measures according to conditions and development stages of patients. During the acute edema period, patients in the observation group should be subject to mental nursing, pain care, training of right lying gestures, and training of getting on and out of bed. Over the pain relieving period, the patients needed to receive lumbar traction nursing, waist care, skin medication, and acu-puncture care. Over the recovery period, patients of the observation group needed to receive lumbar muscle exercises, conducted spine setting-up recovery exercises, accepted health guidance before leaving hospital, and adopted other pro-fessional recovery nursing methods. Observed two groups of patients with symptoms of the disease course of improve-ment, total, total effective rate, 1 year relapse rates, and adverse reactions. Results The observation group patients' symptoms, total treatment and therapeutic effects improved significantly better than that of the control group, ( P<0. 05 ) . Follow-up of 1 year relapse rates was significantly lower than the usual care control group ( P<0. 05 ) . The clin-ical course of treatment had no significant difference in the incidence of adverse reactions. Conclusion Standardized re-habilitation nursing process management can quickly and effectively relieve the symptoms of patients with lumbar disc herniation, increase the cure rate, shorten the rehabilitation time, reduce the recurrence rate, and can significantly im-prove the quality of life and activities of patients with lumbar disc herniation.
