鼻内窥镜下治疗儿童腺样体肥大和分泌性中耳炎的临床研究

目的 探讨鼻内窥镜下治疗儿童腺样体肥大及分泌性中耳炎的方法。方法采用气管插管全麻，104例腺样体肥大患儿行腺样体切割术，其中，43例合并分泌性中耳炎：22例（44耳）显微镜下行鼓膜切开＋置管术，21例（40耳）鼻内窥镜下行鼓膜穿刺抽吸造口术；58例行双扁桃体切除术。结果随访3～6个月，患儿行鼻内窥镜检查见鼻咽部黏膜红润、光滑，未见瘢痕及腺样体残留。所有分泌性中耳炎患儿鼓膜愈合、标志正常存在，鼓室压图正常、患耳听力明显提高。结论鼻内窥镜下治疗儿童腺样体肥大方法简便、切除彻底、创伤小、术后恢复快、并发症发生少。

Search for the method of treating for adenoid vegetation and secretary otitis media under nasal endoscope

Objective To Search for the method of treating for adenoid vegetation and secretary otitis media(SOM) under nasal endoscope.Methods A retrospective study was carried out among 104 children undergone combined tonsillectomy and adenoidectomy from 2001 to 2005 in our hospital.Amony these cases,104 cases was carried out adenoidectomy.43 cases(86 sides) were seen with SOM(41.3%).22 cases(44 sides) were carried out putting middle ear ventilation under microscope.21 cases(40 sides) were carried out puncture and aspiration the liquid in the middle ear and were enlarged the orifice.58 cases were carried tonsillectomy.Results The 104 cases were all cured and there was no complication reoccurred during the period of following up.Conclusion The method is a simple,convenient,micro-trauma and good effection that we carry out adenoidectomy,puncture and aspiration the liquid in the middle ear and enlarge the orifice under nasal endoscope to treat adenoid vegetation and SOM.It is an excellent method to treat adenoid vegetation and SOM.
