摘    要:目的探讨宫腔镜子宫内膜切除术(TCRE)治疗功能失调性子宫出血(DUB)的效果。方法选择行TCRE的DUB病人88例,随访手术疗效,并对术中切除的内膜组织进行病理检查。结果术后随访6~50个月,月经改善总有效率为88.6%;术后贫血纠正率为96.3%;病人对手术效果满意率为85.2%;术后症状复发进行再次干预治疗8例,其中2例行二次TCRE,子宫切除3例,药物治疗3例。8例中4例并子宫腺肌症。结论TCRE是治疗DUB安全有效的方法;TCRE对卵巢功能无明显影响;并子宫腺肌症是影响手术远期成功率的主要因素。

Abstract:Objective To evaluate the curative effect and prognosis of hysteroscopic endometrectomy(HE) for dysfunctional uterine bleeding(DUB). Methods Eighty-eight women with DUB underwent HE were followed up,the operative specimens were studied pathologically. Results Postoperative follow-up was six to 50 months.Overall effective rate of menstruation was 88.6%;anemia-correction was 96.3%,and the patients' satisfaction rate was 85.2%.Eight cases,in which,four associated with adenomyosis,needed reoperation due to recurrence: two underwent repeated HE,three underwent hysterectomy,three received medical treatment. Conclusion Hysteroscopic endometrectomy is a safe and effective treatment for dysfunctional uterine bleeding,it dose not obviously affect the ovarian function.The main factor that influence the long-term success rate of the operation is the disease having associated with adenomyosis.
