宫腔镜子宫内膜切除术治疗功能失调性子宫出血效果

目的探讨宫腔镜子宫内膜切除术（TCRE）治疗功能失调性子宫出血（DUB）的效果。方法选择行TCRE的DUB病人88例，随访手术疗效，并对术中切除的内膜组织进行病理检查。结果术后随访6～50个月，月经改善总有效率为88．6％；术后贫血纠正率为96．3％；病人对手术效果满意率为85．2％；术后症状复发进行再次干预治疗8例，其中2例行二次TCRE，子宫切除3例，药物治疗3例。8例中4例并子宫腺肌症。结论TCRE是治疗DUB安全有效的方法；TCRE对卵巢功能无明显影响；并子宫腺肌症是影响手术远期成功率的主要因素。

HYSTEROSCOPIC ENDOMETRECTOMY IN THE TREATMENT OF DYSFUNCTIONAL UTERINE BLEEDING

Objective To evaluate the curative effect and prognosis of hysteroscopic endometrectomy(HE) for dysfunctional uterine bleeding(DUB). Methods Eighty-eight women with DUB underwent HE were followed up,the operative specimens were studied pathologically. Results Postoperative follow-up was six to 50 months.Overall effective rate of menstruation was 88.6%;anemia-correction was 96.3%,and the patients' satisfaction rate was 85.2%.Eight cases,in which,four associated with adenomyosis,needed reoperation due to recurrence: two underwent repeated HE,three underwent hysterectomy,three received medical treatment. Conclusion Hysteroscopic endometrectomy is a safe and effective treatment for dysfunctional uterine bleeding,it dose not obviously affect the ovarian function.The main factor that influence the long-term success rate of the operation is the disease having associated with adenomyosis.
