椎体终板骨软骨炎的临床和MRI分析

目的 探讨椎体终板骨软骨是的MRI表现及其临床意义。方法 搜集2006年11月-2008年1月间经临床及影像学确诊的终板骨软骨炎167例．男88例，女79例，年龄38-77岁。发生于颈椎者68例，腰椎者91例，胸椎者8例。全部病例均有不同程度的临床症状。分析终板及邻近椎体骨质信号改变。结果 167例共累及312处终板，颈椎140处，腰椎163处，胸椎9处，其中3例病变区较广泛。伴随改变：椎间盘退变428处，突出237处，许莫氏结节162处。按照不同的MRI表现，将终板骨软骨炎分为4型。结论 终板骨软骨炎是造成临床局部疼痛的原因之一，MR是检查本病的最佳方法，临床工作中应注意终板骨软骨炎的存在及其临床意义，将为颈．腰痛疾病的及时诊治提供有益的帮助。

Clinical diagnosis and MRI manifestations of end-plate osteochondritis

Objective To evaluate the MR imaging and clinical significance of endplate osteochondritis. Methods Retrospective analysis was made on 167 patients admitted to hospital from Nov. 2006 to Jan. 2008 including 68 and 91 patients in cervix and lumbar respectively, The patients included 88 male and 79 female, aged from 38-77. All patients showed clinicalsymptoms at various degrees. The signal change in end-plate and its adjacent vertebral bones were analyzed. Results Of 312 cases, 167 patients were diagnosed in end-plate, 140cases in cervix and 163 cases was showed in lumbar. Accompanied changes： 428 cases with lumbar disc degeneration, 237 cases with lumbar disc herniation, and 162 cases in Schmorl＇ s nodule. According to the MRI findings, the end- plate osteochondritis can be categorized into 4 types. Conclusion The end-plate osteochondritis is one reason to induce local pain. In clinical practice, awareness of the end-plate osteochondritis＇ possible existence is of great help in its diagnosis. MRI is the best choice in the imaging detection methods, and can differentiate end-plate osteochondritis from spinal tuberculosis and tumor.
